Job Description Summary: Schedule: 12 hour shifts, 3x a week from 7am-7:30pm, with 5 weekend shifts in a 6 week scheduling period and Works 2 holidays per year on a 3 year rotation Join the OhioHealth Hospice team as a Hospice Registered Nurse at Kobacker House and make a meaningful impact by supporting patients and families through compassionate end-of-life care. Responsibilities And Duties: 90% - Provides direct patient care in accordance with Home Care policies and specified procedures and evaluation of patient/caregiver response to institutions. Demonstrates and provides comprehensive documentation of services rendered. Proficiency in the performance of procedures and equipment. Assesses potential safety issues, both for patient and Home Care staff. Gathers baseline data and records assessment in patients Electronic Medical Records (EMR); identifies problems, develops patient specific goals, and develops a Plan of Care (POC) with collaboration of the intradisciplinary care team based on physician orders and patient collaboration and establishes realistic patient outcomes of care. Continuously assesses patients and changing needs. Notifies physician with any change.
