Description 40 Hours a week Position Summary: Provides clinical knowledge and expertise in coordinating patient care in the Grace Cottage Family Health setting as directed by the medical provider. Collaborate in team-based care within a Patient-Centered Medical Home (PCMH) to deliver coordinated, comprehensive, and personalized care that supports patient wellness and improve health outcomes.
