Transports residents on active therapy program, as well as assists in therapy-related activities assigned by rehab personnel. Responsible for clerical duties assigned by the director and other rehabilitation staff for timely and organized maintenance of equipment and medical records. Duties of an exceptional Rehab Aide (CNA) include: Assists in direct therapy service provision by carrying out delegated, selected patient related tasks under the direct supervision of a therapist allowed by state and federal law. Transports or escorts patients/residents to and from the therapy department safely and efficiently. Provides additional physical help for patient transfer and safety needs under the direct supervision of a therapist or therapist assistant. Maintains cleanliness and safety of the rehabilitation department’s physical environment and equipment. Responsible for maintenance and replenishment of daily needs of clean linen and other commonly used items. Completes assigned clerical duties, such as computer data entry, photocopying, filing, answering the phone, retrieval of therapy-related documentation, and other paperwork. Maintains a record of equipment maintenance and cleaning schedule as assigned by the therapist manager/s. Performs other duties as assigned by his/her supervisor. Exhibits courtesy, compassion, and respect to patients, residents, families, caregivers, visitors, physicians, administrators, and co-workers. Adheres to established confidentiality standards, including but not limited to HIPPA regulations and internal company policies. Rehab Aide Requirements Include: High school diploma or equivalent. Job Summary The Stroke Nurse Navigator is responsible for deploying tactics developed by the leadership team to assist in meeting program goals and objectives of the program. Systematically evaluates the care delivery from pre, intra, and post hospital care (where applicable) to identify opportunities for process improvement and assist relevant process owners with their process improvement. Collaborate with the stroke program coordinator and the outreach nurse to plan and implement materials and programs to support post stroke survivors and their caregivers. All Nurse Navigators act as the primary point of contact and is responsible for monitoring, coordinating care and educating patients and families. Offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers and facilitate timely access to quality health and psychosocial through all phases of the healthcare continuum. Core Job Responsibilities Serves as a resource nurse expert to staff to assist in identifying barriers, locating resources, coordinating services, providing education on stroke; refer patients to integrative therapies and support groups. Creates and maintains open lines of communication with treating physicians, primary care provider, and their support staff; establish referral pathways thereby decreasing treatment delays: working collaboratively with the Neurocritical Care and Neuroendovascular to maintain a network of specialists and support of best practices in care delivery during code strokes and daily rounds; and to participate in stroke conferences. Based upon care delivery observations, makes recommendations for initiatives to improve process and outcome performance and participates in support of those initiatives. Job Summary The Care Manager is responsible for completing a comprehensive assessment to determine the correct services and benefits based on medical necessity for an enrollee. Provides plan education, transition of enrollees to clinical services based on Department of Health (DOH) guidelines. Care management providing advocacy, coordination and oversight of all healthcare services for an enrollee striving to deliver the best care at the right time and in the most-cost effective while meeting quality outcome standards for the enrollee case load assigned.
