Green Hill Seinor Living & Rehabilitation is currently seeking a Part-Time LPN's for day (11am-7pm) shifts to join our dynamic organization. OVERVIEW: Under the leadership of the Registered Nurse, the LPN will be providing coordinated care to residents in a skilled nursing facility. RESPONSIBILITIES: · Provide direct care responsibilities to patients · Supervise CNA’s · Administer medications and treatments timely and according to policy · Receive, transcribe, and carry out physician orders · Follow established procedure for charting · Complete patient assessments, coordination of care · Identify and reports changes in condition · Follows environmental health & safety and sanitation standards and procedures · Oversees clinical and non-clinical acute operations and continuum of care REQUIREMENTS: · Valid LPN License in New York · CPR/BLS certified · 1 + year nursing experience in a hospital, nursing home or other healthcare setting

Responsibilities: Conduct comprehensive psychiatric evaluations and assessments of elderly patients in nursing homes. Diagnose and develop individualized treatment plans based on the patient's mental health needs. Provide medication management and adjust treatment plans as necessary. Collaborate with interdisciplinary teams, including physicians, psychologists, and social workers, to ensure holistic patient care. Offer counseling and psychotherapy services to patients and their families. Educate patients and their families about mental health conditions, treatment options, and self-care strategies. Maintain accurate and up-to-date medical records in compliance with organizational and regulatory standards. Participate in clinical meetings, case conferences, and quality improvement initiatives. Requirements: Valid Nurse Practitioner license in the state of New York with specialization in Psychiatry (PMHNP). Ability to work independently and effectively manage a caseload of patients. Strong diagnostic skills and knowledge of psychiatric medications and treatment modalities. Excellent interpersonal and communication skills to establish rapport with patients, families, and interdisciplinary team members.
