The ARNP will provide comprehensive primary care services focusing on prevention, chronic disease management and health promotion for diverse patient populations. This role requires collaboration with physicians, RNs, pharmacists, social workers, behavioral health specialists and other professionals to optimize the health of MultiCare communitiesResponsibilities Direct Patient Care • Conducts comprehensive assessments, diagnose conditions, develop plans, and prescribe medications for patients across the lifespan • Diagnoses and treats acute and chronic disease using evidence-based guidelines • Provides preventive care, health education and counseling to individuals and families • Timely and accurate documentation, inclusive of direct patient encounters and asynchronous care • Participates in the coverage for clinic colleagues per departmental guidance • Participates in after-hours call coverage per departmental guidance Multidisciplinary Collaboration • Participates in multidisciplinary meetings, care reviews and care planning • Coordinates care with all members of the delivery team • Contributes to the development and improvement of population-based care Population Health Management • Utilizes data and risk stratification tools to prioritize our patients at greatest risk to prioritize outreach • Engages in quality improvement focused on health outcomes, cost effectiveness and cost reduction • Supports transitions of care and reduced readmissions and ED use Citizenship • Demonstrates the MultiCare values of compassion, stewardship, kindness, joy collaboration and respect • Adheres to MultiCare policies and processes Requirements • Maintains active Washington State Nurse Practitioner or Physician Assistant license • DEA, and state prescriptive authority • Current BLS for Healthcare Providers certification by the American Heart Association. Caregiver Job Duties Our caregivers support their clients in activities of daily living such as (but not limited to): Household chores (cleaning, laundry, dishes, etc.) Cooking and/or serving meals Helping clients bathe, dress, and groom Providing companionship through daily activities and hobbies Driving clients (as needed) to the store or appointments Monitoring and reporting on their condition Additional Information Our hiring process is fast and simple! Once you have completed onboarding, orientation, and training, you can start working with clients! If you do not have a current HCA or CNA license, you will be required to enroll in our HCA training program we offer (due to state requirements). You will be able to complete the training while working and will have 120 days to complete it. Requirements 18 years+ Ability to lift a minimum of 10lbs and up to 50lb rarely.
