Job Highlights As a registered nurse care coordinator (RNCC) at UAB Medicine, you'll join a Magnet-designated nursing team to provide the highest quality of care, even for the most complex cases. The UAB Care Transitions Department includes more than 200 registered nurse care coordinators and social workers who are integral members of the interdisciplinary team. RNCCs provide care in both the inpatient and ambulatory settings, including the Emergency Department, with a focus on screening and assessing patients for any post-discharge needs, providing referrals for in-home care, and participating in care coordination and discharge planning for patients across the continuum of care. Qualifications Bachelor's degree in Nursing and three (3) years of related experience required OR Associate's degree in Nursing and five (5) years of related experience required. Work experience may NOT substitute for education requirement. Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required. Certified in Basic Life Support (BLS) from the American Heart Association (AHA) is required or should be obtained within 30 days of employment. Depending on the unit/department, Advanced Cardiac Life Support (ACLS) certification and/or Pediatric Advanced Life Support (PALS) from the American Heart Association (AHA) may be required. **Caregiving Agency: Helping Hearts USA** We are currently hiring dedicated caregivers to provide 24/7 assistance to clients. Available shifts are from 7 AM to 7 PM and 7 PM to 7 AM. The client is located near HWY 119 and HWY 280, close to Dick's Sporting Goods. Compensation for this position ranges from $13 to $14 per hour. Candidates must have experience with the following tasks: toileting, showering, changing briefs, assisting with ambulation, running errands, driving clients to appointments, meal preparation, and light housekeeping.
