About this role The Licensed Practical Nurse (LPN) plays a vital role in patient care by contributing to the nursing process of assessment, planning, implementation, and evaluation. What you bring to the table Education Graduate from an accredited school of nursing. Experience Prefer a minimum of one (1) year clinic experience Licenses/Certifications Have and maintain current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment. Must be able to obtain and maintain CPR certification. What you will do Under RN supervision, assesses patients' physical, psychological, and social dimensions in both the Health Care Unit and housing units, contributing to individualized treatment planning and consulting with RNs and staff as needed. Implements treatment programs directed by Health Care Practitioners, administering medications, including IV therapy (if certified), and performing diagnostic tests according to established protocols and procedures. Collaborates in controlled substance counts at the beginning of each shift, adhering to safety techniques, procedures, and standard precautions while responding to codes or health emergencies within standard guidelines. Assists in medical or minor surgical procedures, implements nutrition and therapeutic diet plans, and documents nursing encounters using SOAP charting, ensuring effective communication with nursing staff, physicians, and other relevant personnel. Attends mandatory staff meetings and training sessions, demonstrating a commitment to continuous professional development and adherence to policies and procedures in providing high-quality patient care. Job Description The Medication Tech / CNA @ The Cardinal at North Hills is responsible for the delivery of quality care by encouraging independence and ensuring each resident’s physical, social, cultural, and intellectual well-being. Full Time / Part Time 1st (7a-3p) 2nd (3p-11p) 3rd shift (11pm-7am) Medication Technician **Must have a CNA certification** ** Pay Range $21/hr - $23/hr CNA ** Pay Range $17/hr - $20/hr **based on certifications & experience Openings are in our Skilled, Memory Care & Assisted Living Units. Observe residents for any changes in condition and report to supervisor Assist residents with simple requests as needed Document and report changes of condition Provide medication set up and assistance Log in medications from pharmacies, verify physician’s orders and communicate any concerns Track all medications and re-order as needed Track residence vital signs, diabetic testing and/or insulin injection, ambulation, etc. What will I need to be successful in this role? The Medical Support Specialist (MSS) is responsible for performing routine medical and testing procedures under direct supervision of the treating clinician as well as supports in other areas of the facility as needed including registration and referral management. This role requires efficient and accurate delivery of one-on-one patient care while thriving in a fast-paced environment. The MSS ensures that every patient is treated the with quality clinical care and receives an excellent patient experience from welcoming, respectful, and skillful colleagues. Responsibilities Perform Department of Transportation (DOT) and Non-DOT drug and alcohol testing Prepare patients for physical examinations, including taking vital signs and any required or requested ancillary testing including but not limited to vaccines and injections, phlebotomy, PFTs, audiogram testing and respirator fit testing Perform rapid screening tests (influenza, strep, mono, glucose, etc.) Assist providers during examination and treatment Responsible for performing routine medical procedures as ordered by treating clinician as well as triaging emergent patients as needed Prepare and assist clinician with procedure set up and injury care Apply bandages, dressings and splints as ordered by the treating clinician Dispense medications and DME as ordered by the treating clinician in accordance with state regulations Complete quality assurance activities on equipment and medical devices as well as testing processes used in the center Maintain supplies, clean rooms and equipment, and stock exam rooms Maintain and operate all medical devices and equipment, including calibration and record keeping in accordance with current policy and procedures Notify supervisor immediately if equipment is not functioning properly, supplies are needed, or facility is not operating as expected In partnership with center leadership, assist with patient flow and volume Keep patients informed of expected wait times during all aspects of the center visit Responsible for clerical tasks in both the front and back office associated with patient care and proper record keeping Ensure accuracy in documentation Perform front office duties: greet patients, obtain authorization, check in/out, communicate wait times, answer multi-line telephone system, and distribute employer results/paperwork as needed Follow HIPAA guidelines and safety rules Attend center staff meetings or huddles as required Assist in maintaining a neat, clean, and orderly appearance throughout the facility Complete any applicable training including but not limited to clinical competency training that occurs monthly This job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job.
