Overview The Community Outreach Population Health Nurse Practitioner (NP) will provide primary health care and behavioral health services as a member of a healthcare team comprised of Behavioral Health and Community Outreach RNs, Peers, Community Health Workers, and drivers. In this role the NP works on our mobile health clinic (MHC) that travels throughout Sussex County. The MHC has a focus on Behavioral Health and Addiction Medicine. Responsibilities Clinical Responsibilities: Comprehensive patient care, including assessment, treatment planning and implementation, counseling and education of patients and their families. Overview This individual is a clinically competent Cardio-Vascular Advance Practice Clinician (APC) will directly report to the Director of APCs with a matrix reporting structure to the Executive Director of the Cardiac and Vascular Service Line.
