Responsibilities: Follows nursing assistant plan of care established by the Registered Nurse within a specified timeframe; including, but not limited to, mouth care, bathing, AM and PM care, assistance with ambulation, transfers to chair and/or bed, taking and documenting vital signs, and recording accurate intake and output. Documents all patient care delivered according to established policies and procedures. Performs duties with the knowledge and skills necessary to provide care appropriate to the age of patients served on the assigned units. Maintains a safe, comfortable, and therapeutic environment for patients and families in accordance with hospital standards. Communicate relevant patient information to a professional nurse within an appropriate time based on the level of urgency. Report problems immediately to a professional nurse. Answer intercom or call lights in a timely, courteous manner, with appropriate communication to the patient and/or professional nurse. Performs patient care based on established guidelines for diagnostic and therapeutic exams; i.e., NPO for labs or x-rays, OR, diet changes related to tests, enema-related tests. Perform safety and suicide precaution sitter duties as assigned. Assists other members of the nursing team and/or ancillary departments as requested. Professional nursing practices, requiring specialized knowledge judgment and skill are performed using the process in accordance with hospital policy, procedures and nursing philosophy. Responsibilities: Provide direct, professional nursing care to specified population of patients in the acute care setting. Develops a patient centered plan of care based on individualized needs. Maintains confidentiality of the patient medical record. Counsel and educate patients and their care partner on treatment plans. Administer medication following the five rights of medication administration. Provide wound care and dressing changes, as applicable. Read and interpret patient information, making treatment decisions, when appropriate. Consult with physicians to decide on best treatment decisions when appropriate Performs nursing interventions to achieve optimal patient outcomes. Observe and record patient behavior and response to nursing interventions. Demonstrates quality, safe and effective patient care. Provides delegation to unlicensed assistive staff and Licensed Practical Nurse (LPN) staff Demonstrates standards of excellence which includes practicing nursing with core values of compassion, accountability, respect, teamwork and integrity. Demonstrates understanding and executes quality and performance improvement plans. (only for RN evaluation) Participates in decision making and takes actions based on ethical principles. (only for RN evaluation) Provides timely and accurate documentation of patient care. All other duties as assigned within the scope and range of job responsibilities. Required Education, Credential(s) and Experience: Education: Associate Degree Nursing Or Nursing Diploma.
