Job description Cordial Health is seeking a dedicated and compassionate Psychiatry Nurse Practitioner (PMHNP) to join our team. Responsibilities: Conduct comprehensive psychiatric evaluations and assessments of elderly patients in nursing homes. Diagnose and develop individualized treatment plans based on the patient's mental health needs. Provide medication management and adjust treatment plans as necessary. Collaborate with interdisciplinary teams, including physicians, psychologists, and social workers, to ensure holistic patient care. Offer counseling and psychotherapy services to patients and their families. Educate patients and their families about mental health conditions, treatment options, and self-care strategies. Maintain accurate and up-to-date medical records in compliance with organizational and regulatory standards. Participate in clinical meetings, case conferences, and quality improvement initiatives. Requirements: Valid Nurse Practitioner license in the state of New York with specialization in Psychiatry (PMHNP). Ability to work independently and effectively manage a caseload of patients. Strong diagnostic skills and knowledge of psychiatric medications and treatment modalities. Excellent interpersonal and communication skills to establish rapport with patients, families, and interdisciplinary team members. Job Title: Registered Nurse Department: Med-surg Location: Massena Hospital Hours Per Week: Temporary full-time Schedule: 12-hour day shifts SUMMARY: The registered nurse provides assessment, care planning and direct delivery of treatment utilizing professional nursing care processes in a patient and family centered manner in accordance with licensure and regulatory scope and standards, and established hospital policies, procedures and protocols. RESPONSIBILITIES: Performs a comprehensive patient assessment to identify patient and family needs. Develops and implements a comprehensive plan of care involving the patient and the family. Performs and documents direct patient and family centered care and patient education according to nursing standards of practice. Identifies and responds to patient conditions to achieve positive patient outcomes. Performs discharge planning to ensure safe and appropriate transition of care. Directs and delegates care provided by unlicensed team members. PAY RANGE: $42.41 - $57.24 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
