Job Title: Registered Nurse (Per Diem) - Outpatient Dialysis Location: Unity Hospital Hours Per Week: 0 Schedule: Monday-Friday Days/Evenings (5:30am-10pm)plus 2 Saturdays per month OUR Department: Rochester Regional Health offers a full range of Dialysis treatment options to patients with chronic kidney disease. We treat adults from people from multiple backgrounds, cultures and ethnicities. We care for patients temporarily, either their kidney function recovers or they return to their home dialysis center, move or get a transplant; or some are with us on an ongoing basis. Patients have dialysis treatment anywhere from 2-3 days a week, from 3 to 5 hours each treatment. Our staffing model consists of RN’s, LPN’s and Techs. Job Title: Licensed Practical Nurse - Acute General Medicine Department: Acute General Medicine (5800) Location: Rochester General Hospital Hours Per Week: Full-Time (36 hours per week) Schedule: Day/Evening/Night, Every Other Weekend, Every Other Holiday SUMMARY: The Licensed Practical Nurse is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans. OUR UNIT: 4400 is an 18 bed, Oncology/Palliative Care unit. The unit cares for oncology, palliative care, and hospice patients. The nurses are required to become chemotherapy certified through Oncology Nursing Society (a nationally recognized certification). RRH reimburses or pays for this certification for our team members. Staff are also trained to use and care for patients on CADD pumps, used for patients who require high doses and increased frequencies of pain medication. These are seen for a variety of reasons but are commonly seen with sickle cell and comfort care patients. 4400 has achieved the AACN GOLD Beacon award for excellence. 4400 is also a DNV Palliative Care Certified unit. Job Title : LPN Team Case Assistant DEPARTMENT : Home Care LOCATION : Rochester STATUS : Full Time SUMMARY Utilizing clinical experience, coordinates patient services under the direction of the team manager/designee. This includes but not limited to prioritization and coordination of daily patient schedules, requesting additional services such as aide, therapy, social work, interpreter services, and ensuring that referrals for services are appropriately assigned in a timely fashion.
