Description Staff Nurse - Surgery Administration (Outpatient) - Mount Sinai West - Full Time Day 9am to 5pm Responsible for the care activities and goal achievements of assigned patients by assessing, diagnosing, planning and intervening in actual or potential health problems and evaluating patient's response to care. Responsible health teaching and discharge planning. Acts as a member of a designated care team and collaborates, coordinates and guides team members in planning patient care activities. Demonstrates behaviors, which reflect the scientific knowledge, judgment and teaching skills of a professional nurse. Responsibilities 1. Conducts Nursing assessment based on interview, examination, observation and review of records of the patient 2. Ensures that the patients progress, or lack of progress, is reflected in progress notes and in the revised care plans 3. Discharge planning consistent with the patients status and reflects inter-departmental and inter-agency communication 4. Communicates information to the patient based on the patients level of awareness and readiness to learn 5. Supports orientation of new staff members by observing professional behaviors, providing feedback to the staff member and by acting as a preceptor, role model and a resource person 6. Functions as a professional role model and mentor for students and volunteers. 7. Patient care assignments reflecting an awareness of patient needs and skill level of personnel 8. Organization of provision of care to meets priority needs of patients and unit 9. Communicates condition of patients and unit requiring additional intervention to the leadership person in the relevant discipline 10. Participates in the development of other staff members 11. Communicates with peers in an appraisal of practice as it relates to self and other nursing personnel 12. Reflects a collaborative effort in meeting the needs of the patient and unit through communication and nursing actions 13. Provides direction, assistance and support 14. Assumes accountability and responsibility for completion and quality of assigned tasks 15. Identifies and communicates to immediate supervisor the need for further staff education 16. Reflects awareness of authorities and responsibilities of nursing management levels by communication with nursing leadership 17. Bachelor’s Degree in Nursing. (BSN) Current New York State Registered Nurse/ RN license (or willingness and eligibility to obtain) Associate’s Degree in Nursing (ADN) RN who is matriculated in a BSN Program with a graduation date within the year. Any Associate’s Degree in Nursing (ADN) RN with Nursing experience must be approved by the CNO. Documented previous experience or educational preparation to support credentials. Specials skills/experience may be required in specialized areas. Wound and Ostomy care experience a plus. Prepares patients for meals including: Clearing overbed tables and placing them within reach of the patient. Positioning patient correctly for eating/tube feedings. Assisting patients by preparing utensils and food items. Feeding patients. Performs and/or assist patient with simple treatments. Remove and reapply SCD wraps as directed by the RN. Assists RN in enema procedures. Collects specimens as directed by RN. Informs RN of any audible clinical alarms such as IV pumps, feeding pumps, ventilators, bedside monitors Assists with moving, lifting, positioning and transporting of patients. Assists with removal and security of patient personal items prior to procedures, as needed. Changes linen in accordance with policy and as necessary. Takes and electronically records blood pressure, temperature, pulse, pulse oximetry, and respiration per care level standard and as directed by the RN. Reports abnormalities to RN. Weighs patients and electronically records on admission, as ordered, and/or as directed by RN. Measures and records oral intake, urinary output, bowel elimination, gastric and other drainage onto I&O record (form or electronic). Calculates 8 hour totals. Empties drainage bags. Performs post mortem care. Performs safety observations for patients on continuous observation or in restraints, per policy and as directed. Reports any changes observed in the patients condition to the RN. Responds to and meets patient/family needs and customer concerns as soon as possible with compassion and understanding. Established effective, caring relationships with staff, patients, family and other members of the health care team. Maintains competence through in-service and educational programs. Attends staff meetings and reviews/signs minutes of meetings not attended. Maintains a safe and uncluttered environment of care in patient rooms including removing excessive furniture, supplies and equipment. Assists with supply and equipment management of patient, procedure and supply rooms per department standards. Maintains cleanliness of utility rooms and pantries as assigned. Performs other duties such as errands, relieving other units, bringing stretchers and assisting other members of the health team, as directed.
