The Primary Care Mental Health Program provides a team-based approach to healthcare by integrating mental health providers and registered nurses into Primary Care Clinics to deliver outstanding, evidence-based biopsychosocial assessment, treatment, prevention and management of mental health problems and biopsychosocially influenced health conditions for Military Health System beneficiaries. 1. Care Facilitation and Communication The Mental Health Care Facilitator performs independent, comprehensive telephone and clinical triaging in the primary care setting, supporting primary care providers in their treatment of patients with mental health conditions. Facilitates continuity of care through a multidisciplinary approach in the treatment of depression, post-traumatic stress disorder (PTSD) and anxiety disorders in adult beneficiaries. Monitors patient progress and symptom severity by conducting a structured telephonic interview and using validated tools (e.g. PHQ-9, GAD-7 and PCL-5). Provides patient education as an integral part of the services offered (e.g. medication education, alcohol effects on mood, etc.). Collaborates with patients to develop SMART self-management goals to assist patients with symptom improvement. Assists patients with treatment plan adherence.
