We are seeking a After Hours Registered Nurse (RN) to provide critical support to patients and families across the Blue Ridge Care territory during evenings, nights, weekends, and holidays. Position Summary The Registered Nurse After Hours (RN), in accordance with public health nursing standards, practices, and policies, will provide and arrange for hospice care for patients and families in collaboration with other members of the interdisciplinary team. The RN makes initial assessments, judgments, and treatment decisions based on patient and family needs and wishes, in alignment with professional skills and hospice philosophy. Nursing Care Provide professional nursing care consistent with patient care goals and the physician’s plan of care. Assess and evaluate patient status; if working with LPN support, provide guidance and oversight. Care Planning and Implementation Develop and update care plans with goals based on nursing diagnoses. Administer medications and treatments as prescribed, including preventive and rehabilitative care. Patient and Family Counseling Counsel patients and families on health care needs, offering education and guidance. Teach self-care techniques, including medication administration, diet, and symptom management. Documentation and Communication Maintain accurate and timely clinical documentation of patient condition and care. Communicate with physicians regarding patient changes and obtain/document orders. Environment & Equipment Management Ensure safe patient environments and arrange necessary equipment/services. Continuity of Care Conduct initial assessments and collaborate with patients, families, and the care team to ensure seamless care transitions. Professional Development & Quality Improvement Engage in continuing education and quality improvement activities. Support initiatives like peer chart reviews, medication reconciliation, and tuck-in programs. Education & Experience Current RN license in the Commonwealth of Virginia Graduate of an accredited nursing program; BSN preferred 2+ years of experience in hospice, home health, or ICU/CCU (preferred) CHPN certification (preferred) Knowledge of Medicare/Medicaid reimbursement regulations Current CPR certification Strong communication and interpersonal skills Proficiency in electronic medical records and computer skills Ability to travel throughout the Blue Ridge Care territory Weekend availability only will be considered. The RN would be on call from Friday at 4:30 pm until Monday at 8:00 am (eastern). The RN would be responsible to cover any as-needed visits that arise, as well as scheduled weekend visits including admission visits. Compensation will include an on-call stipend PLUS pay per visit for each visit completed. The Nurse will be responsible for assessing patient health problems and needs, developing and implementing nursing care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration is required.
