Responsibilities: Utilize the nursing process to assess, plan, implement, and evaluate patient care. On each assignment, provides skilled nursing care/services in accordance with prescribed orders Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition. The RN Coordinator Practitioner Excellence will work in close collaboration with Medical Staff leaders and other members of the Medical Staff Office on providing accurate clinical summaries, data trends, and providing detailed reports for analysis when indicated. This position will also monitor significant trends by analyzing aggregate data and monitoring the performance of all practitioners, identifying areas for performance improvement and the quality of care we provide. This includes clinical performance and escalating professional behavior concerns to the Director Practitioner Quality and Professionalism. The RN Coordinator Practitioner Excellence will be responsible for coordinating and managing all Medical Staff Peer Review meetings. ESSENTIAL DUTIES AND RESPONSIBILITIES – Other duties may be assigned. · Identify cases meeting the criteria for clinical case review through pulling reports from various data systems and provide a preliminary analysis (RN level) of findings. · Collect data as specified in the Professional Practice Evaluation (PPE) Policy for aggregation, analysis, and presentation including an annual summary to be presented to the Medical Board by Medical Staff Leadership. · Collect data, as requested by hospital committees as defined in the PPE-Policy including but not limited to Physician Excellence Committee, Hospitalist Quality and Process Improvement Committee or others for ongoing trends or outlier status and to identify opportunities for improvement. Create charts and graphs to facilitate the communication of data trends. · Maintains a file on each practitioner for tracking and trending variances through the peer review database. · Creates and maintains records of the peer review discussion and outcomes. · Maintains a close working relationship with the Committee Chairpersons and members. · Follows the Peer Review Process as outlined in the Medical Staff PPE Policy and the Medical Staff Organization and Functions Manual. · Prepare a summary of all the medical records and other relevant data sources to be reviewed by Medical Staff Committees. · Responsible for setting the agenda, creating the presentation material, and recording minutes for the Medical Staff Committee meetings. · Attends Medical Staff meetings to facilitate the peer review process as designated. · Coordinate follow-up planning processes and activities including system opportunity initiatives from the recommendations from PEC, its members, or any of its subcommittee meetings. · Coordinates activities with all hospital departments involved in data collection, scheduling of meetings, and interfaces with the Medical Staff Services Department to facilitate communication and appropriate meeting support. · Expediate timely and accurate triage of Medical Staff related Risk Events Notices, including variance reports, patient grievances, and Amendment 7 requests. · Knowledgeable of Evidence-based, CMS and TJC clinical metrics (i.e. Core Measures, AHRQ Patient Safety Indicators, Hospital Acquired Conditions) · RN Coordinator Practitioner Excellence reports directly to the Director Practitioner Quality & Professionalism EDUCATION, EXPERIENCE AND QUALIFICATIONS · Graduate from an accredited school of nursing, BSN required. · Licensed as a Registered Nurse (RN) in the State of Florida. · Strong clinical background with a minimum of three years of recent acute care experience with a minimum of two years in Performance Improvement preferred. · Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows.

JOB SUMMARY The Registered Nurse independently plans, implements, and evaluates nursing care in patient care situations using standards of care, policy, and procedure. Provides direct nursing care to patients, assumes teaching and leadership responsibilities. Participates in shared decision making, unit committee and quality improvement activities. Needs minimal supervision. ESSENTIAL DUTIES AND RESPONSIBILITIES – Other duties may be assigned. · Develops plan that includes strategies to attain expected outcomes. · Implements the plan of care interventions and medical plan of care as appropriate. · Coordinates care delivery. · Employs patient teaching to promote health and safety. · Evaluates progress toward outcomes. · Practices ethically. · Attains knowledge and competence that reflects current nursing practice. · Integrates evidence and research finding into practice. · Contributes to quality nursing practice and shared decision making (SDM) by supporting council decisions, implementation of decisions and active participation in SDM. · Communicates effectively in all areas of practice. · Demonstrates leadership in the professional practice setting. · Collaborate with patient, family, the inter-professional team, and others in the conduct of nursing practice. · Utilizes appropriate resources to plan and provide nursing services that are safe, effective, and financially responsible. · Practices in an environmentally safe and healthy manner. · Accountable to foster, participate, and be knowledgeable with the shared decision-making model of the NCH Healthcare System. EDUCATION, EXPERIENCE AND QUALIFICATIONS · Licensed as a Registered Nurse (RN) or Advanced Practice Registered Nurse (APRN) in the State of Florida. · Basic Life Support (BLS) certification required from the American Red Cross or American Heart Association. · All new to practice RN will be placed into the RN Residency Program. · Intermediate computer knowledge: Uses Microsoft Word, Excel, Outlook, and Windows
