Our Company ResCare Community Living Overview We are seeking an experienced and licensed Registered Nurse (RN) in Texas to complete Comprehensive Nursing Assessments (CNA) in compliance with Texas Health and Human Services (HHSC) requirements for individuals receiving community-based services. The RN will conduct in-home assessments, document health needs, and develop service plans that promote the health, safety, and independence of individuals served. Responsibilities Conduct Texas Comprehensive Nursing Assessments (CNA) in accordance with HHSC guidelines. Evaluate the individual’s physical, medical, and psychosocial status through observation, interview, and review of medical records. Maintain accurate, timely, and complete documentation of assessments and follow-ups. Conduct reassessments as required (annually or upon significant changes in condition). Maintain confidentiality and adhere to HIPAA and agency policies. Qualifications Current, unencumbered Texas RN license (required). Minimum of 2 years clinical nursing experience; community-based, long-term care, or home health experience preferred. Strong knowledge of Texas Comprehensive Nursing Assessment requirements and HHSC documentation standards. Must be willing to obtain professional liability insurance. Ability to work independently, manage time efficiently, and meet strict deadlines. Excellent communication, clinical judgment, and interpersonal skills. Must have reliable transportation and willingness to travel to client homes. Qualifications POSITION SUMMARY Responsible for the timely processing of inquiry and referral calls. Responsible for coordination of all admissions processes and serves as liaison to referral sources, families, payors, physicians, unit and hospital personnel involved with each admission. Verify insurance, triage patients, complete assessments, ability to staff clinical effectively, enter calls and admissions in computer and complete all admission paperwork with patient. RN - Admissions Requirements: Required: Graduate of accredited School of Nursing; Bachelor Degree; Preferred: Master's Degree Required: Minimum of one (1) years in the area of substance abuse/psychiatric treatment programs, Preferred Minimum of two (2) years’ experience in psychiatric treatment; previous experience in intake/admissions and managed care. Working knowledge of DSM-V diagnostic criteria, mental status exam, and addiction and recovery field. Required : Registered Nurse with active license and able to work in the state of Texas Additional Requirements: CPR certification, Handle with Care and Verbal De-Escalation training within 30 days of employment prior to assisting in a restraining procedure. Training will be offered during new hire orientation. May be required to work flexible hours and overtime.
