Assists in the delivery of primary health care and patient care management by performing selected clinical and administrative duties. Duties And Responsibilities Assists provider in preparing for physicals and minor surgeries. Prepares patients for examination and treatment. Takes patient histories and vital signs, performs phlebotomy and EKG as needed. Prepares exam and treatment rooms with necessary instruments. Assists with ordering and maintaining supplies and equipment, including sterilization of instruments. Assists with scheduling of tests and treatments and follows through with referrals when indicated. Screens telephone calls for referral to nurse or provider. Maintains patient files, records and other information according to office standards. Implements provider orders under provider direction and notifies patients of results as requested. Remains updated and current on the electronic health record policies and procedures. Attends required meetings and participates in committees and professional development activities. The Nurse Case Manager must be a registered nurse with an active license to practice nursing in the State of Connecticut who has at least one (1) years' nursing experience in an acute/sub-acute or home care setting. Other Qualifications Proof of completion of an Infusion Therapy Education Program preferred Current certification in Cardiopulmonary Resuscitation from American Heart Association or American Red Cross Must possess a valid driver's license and personal vehicle. Physical, mental and sensory abilities sufficient to meet performance criteria with or without assistive devices. Performance Criteria The Nurse Case Manager will possess the following skills and attributes to be responsible for providing direct care, health counseling and teaching to agency patients. Implements the physician ordered plan of care using available resources. Obtains physician or dentist orders for all changes in treatment and medical supplies. Obtains insurance authorization as needed for reimbursement. The Nurse Case Manager partners with others to effect change and produce positive patient outcomes Orients and supervises professional and paraprofessional personnel providing patient services. Communicates information regarding each patient's health status and plan of care among all relevant care providers, including the home health aide and the physician or dentist. Information may include but not be limited to: Patient condition, response to interventions and teaching, changes in plan of care and discharge planning. Reports abnormal findings or additional needs to the physician or dentist in charge of patient's care. Makes referrals to other disciplines when indicated and ordered. Provides supportive and restorative nursing care and patient counseling and teaching in accordance with agency policy and procedure and accepted standards of practice. Adheres to infection control and exposure prevention practices as outlined by agency policies and procedures. Scope Of Position The critical care resource nurse serves as a clinical, educational, and professional resource within the organization, providing advanced nursing expertise to support staff and optimize patient care outcomes. This position requires expert knowledge and clinical nursing skills in the management of the critically ill patient population. The resource nurse will function as a role model, mentor, and consultant to nursing staff and interdisciplinary teams, supporting quality care and evidence-based practice. This position reports to the Director of Nursing Education. Responsibilities Serves as clinical expert and organizational resource to maintain, enhance, and advance nursing practice in critical care. Validate and evaluate staff competencies and skills based on employee specific needs assessment. Identify, communicate, and collaborate with clinical director, manager, and educator regarding competency gaps . Facilitate efficient on-going patient care and workflow on unit, participating in functions as needed. Respond promptly to hospital wide emergencies including but not limited to Code 3, Rapid Response, and Stroke Alert. Transport monitored patients for procedures and diagnostic testing. Assist with coordination of ICU admissions, discharges, and transfers to ensure appropriate patient placement and continuity of care. Troubleshoot, evaluate, and support the safe use of advanced equipment. Collaborate with interdisciplinary team to align patient condition Facilitates understanding of skills and processes for staff and students, formally or informally, to enhance their ability to provide patient care. Works with the Medical Care Team to facilitate patient flow and appropriateness of level of care to patient condition. Support clinical decision making. Maintains and develops own clinical expertise and knowledge in specialty nursing practice. Performs sepsis alert reviews Performs other duties as assigned in alignment with organizational needs. Requirements Bachelor of Science in Nursing (BSN) required; Master’s degree in Nursing or related field preferred. Current CT RN license in good standing. Current BLS and ACLS certifications (must be maintained). Maintenance of the above required licenses and/or certifications Three (3) years minimum of critical care experience required. Previous/current preceptor and charge nurse experience preferred. Excellent clinical assessment and critical thinking skills. Strong communication, interpersonal, and team collaboration skills. Ability to manage and troubleshoot advanced medical equipment. Working knowledge of critical care protocols, procedures, and best practices. Evidence of on-going continuing education, experience, and competency in the acute care setting. Act as resource for aides and students for all procedures in the ED and on call. Demonstrate the skill and judgment necessary to implement Physician's orders, nursing interventions and procedures as necessary for the care of the patient. Maintain up to date and accurate documentation of nursing care provided to insure the integration of information for use by the health care team and to insure the totality of quality care. Promote effective working relationships and work cooperatively as part of the department team to facilitate the department's ability to meet its goal and objectives. Contributes to Quality Improvement activities and participates in the departmental Quality Improvement Program. Responsible for own professional development to insure appropriate knowledge of advances in practice to provide state of the art treatment for patient population serviced.
