Under supervised professional and skilled guidance, our CNAs give direct nursing care to assigned residents and perform other services necessary for their personal and comforts needs. We look forward to meeting with you to discuss professional opportunities at our facility. Job Responsibilities / Duties: Promotes resident hygiene and comfort. Distributes food/water and feeds residents. Assists with repositioning and transferring residents. Ambulates and transports residents. Records and totals fluid intake and output. Weighs residents. Documents on flow sheets. Assists with admission, discharge and postmortem care. Responds to call lights in a timely manner. Performs perineal care and uncomplicated colostomy care. Reports all observations & assessments to the Team Leader/Nurse Manager/Charge Nurse. Takes and records vital signs. Makes beds and keeps resident’s units clean and in order. Prepares room for housekeeping after resident discharge. Skill / Experience Requirements . Current NYS CNA Certification Required. Previous Long Term Care experience a plus. Responsibilities: Conduct comprehensive psychiatric evaluations and assessments of elderly patients in nursing homes. Diagnose and develop individualized treatment plans based on the patient's mental health needs. Provide medication management and adjust treatment plans as necessary. Collaborate with interdisciplinary teams, including physicians, psychologists, and social workers, to ensure holistic patient care. Offer counseling and psychotherapy services to patients and their families. Educate patients and their families about mental health conditions, treatment options, and self-care strategies. Maintain accurate and up-to-date medical records in compliance with organizational and regulatory standards. Participate in clinical meetings, case conferences, and quality improvement initiatives. Requirements: Valid Nurse Practitioner license in the state of New York with specialization in Psychiatry (PMHNP). Ability to work independently and effectively manage a caseload of patients.
