The Case Manager has a thorough understanding of OASIS documentation and plan of care development, and their impact on 5 Star Agency Performance and Value Based Purchasing performance. Qualifications REQUIRED QUALIFICATIONS: Education: Completion of nursing program Experience: Experience as RN in a healthcare environment for two years or more. Licensure/ Certification: RN License in IL or MI depending on state they are working in. Current American Heart Association HealthCare Provider BLS is required prior to start date of employment.
