The Phone Triage Registered Nurse provides skilled nursing telephone triage for participants experiencing medical concerns during nights, holiday and weekends in accordance with InnovAge policies and protocols. The Phone Triage RN works in collaboration with the PACE center staff. This role provides nursing support to the participants, caregivers and staff after hours via phone and communicates with the provider on call. Provides clinical guidance and education to the participant, family and caregivers as needed. Responsible for facility transition planning when on call in cooperation with the clinic team.
