About the unit 6 North is a 32-bed surgical floor that offers a great learning environment fostered by our team of exceptional nurses. 6 North offers experiences that are normally not found in Med-Surg clinical rotation. The unit has a focus on surgical conditions including urology procedures, major and minor abdominal surgeries, ENT, bariatric and pulmonary surgeries, peripheral vascular procedures as well as other general surgical post-op needs. The unit has 8 telemetry beds for patients requiring cardiac monitoring. It is a fast-paced environment that you will never find boring. About the unit 2 East is a Surgical Critical Care Unit, specializing in cardiovascular surgical interventions. We also provide care to patients requiring general surgical interventions. Our nurses possess skills that allow them to demonstrate immense critical thinking and physical assessment skills which sets them apart as they continually manage acutely ill patients. Job Description: This is a clinical role. The Advanced Community Care Clinician is responsible for delivering medical care in the communities where Cityblock members live. In homes, shelters, offices, public places or in the street. The ACCC/AC3 is primarily responsible for facilitating virtual provider visits with Cityblock members and following medical orders, performing clinical skills and tasks within their scope, training, and policy. Which may include, IV insertion, IV/IM/SQ medication administration, 12 EKG, Phlebotomy, etc. (Dependent upon local legal regulations). Responsibilities: The Advanced Community Care Clinician will play a vital role in delivering this model of care by making sure they are well equipped, operationally ready, certified, trained, and effective. Post visit they will complete documentation, process lab work in a centrifuge and decontaminate their equipment. They will then get themselves set for the next visit by restocking and preparing equipment. Their vehicle whether Cityblock owned or personal; will be kept, maintained and used in accordance with company policy.
