Our highly trained physicians and nurses have advanced training, specializing in advanced heart failure, complex medical needs of heart transplant, and VAD patients. The RN Navigator is responsible for coordination of care across the spectrum of advanced heart failure with strong emphasis on patient education and engagement in care, including self-management. This employee will implement and monitor patient care plans, monitor, record and communicate patient condition, and utilize a multidisciplinary approach to patient care. This employee will note and carry out physician and nursing orders, provide efficient and effective telephone triage for the heart failure patient population and serves as the primary contact and resource for patient/family throughout the continuum of care. They will contribute expertise as a member of a multi-disciplinary team to facilitate the provision of care of those patients with advanced heart disease and serve as a liaison between physician and other disciplines (VAD and Transplant Team) to expedite interventions which optimize clinical outcomes[ Essential Functions: Completes comprehensive patient health evaluations in collaboration and consultation with the Cardiologists, Specialists and Multidisciplinary Team Uses all components of the nursing process, to provide comprehensive nursing care, support and education to patients and their families regarding screening and diagnostic tests, and treatment of advanced heart failure Responsible for the coordination of care from initial referral through the care continuum including referral for advanced therapies such as mechanical circulatory support and heart transplant Staffs outpatient heart failure clinic, schedules follow-up visits, and triage’s patient calls. Provides ongoing education and reinforcement to patients and caregivers regarding medication regimen, dietary compliance, follow up, etc Other Functions: Coordinates the long term follow-up care of each heart failure patient Schedules follow-up laboratory tests, X-rays, cardiac catheterizations, etc. according to the established protocols Reviews all test results and reports any abnormal findings to the appropriate physician Triages phone calls from heart failure patients regarding medical problems, questions/concerns, medications, etc. and provides the appropriate follow up Communicates with other team members regarding problems/concerns to ensure continuity of care and patient safety is maintained Liaises with other members of the multidisciplinary team to ensure psychosocial and financial needs of the patient are being addressed Maintains accurate and current documentation of demographics, To Dos, laboratory, diagnostic tests, medications, clinic visits, telephone follow up, other miscellaneous communication, etc. in the EMR Assures the evaluation of the CHF candidate is complete Initiates evaluation protocol at the time of referral including assessment of patient acuity to determine level of urgency in expediting the evaluation process. Schedules all clinic visits according to established protocols and the needs of the patients. Performs nursing assessments on each patient with documentation of findings. ESSENTIAL FUNCTIONS : Assesses, develops and implements total nursing care for the pre-operative patient Records and reports to appropriate persons all symptoms, reactions, abnormalities and changes in the physical and/or mental conditions of the patients Administers medications and treatments ordered by the physician. Cares and discharges pre-operative patients according to facility policies. Reviews the pre-operative patient’s chart for accuracy and completeness • Prepares the operating room for surgical procedures, ensuring all equipment and supplies are available and functioning properly • Assists the surgical team by circulating cases and maintaining a sterile and safe environment • Performs surgical time out and verifies patient, procedure, and site according to policy • Communicates effectively with surgeons, anesthesia, and team members during procedures • Documents intraoperative care accurately in the medical record • Monitors patient safety throughout the procedure and responds to changes in patient condition • Ensures proper handling, labeling, and disposition of specimens • Assists with room turnover to maintain efficient case flow • Adheres to infection control, safety, and regulatory standards including AORN and facility policies Ascertains surgical consent is completed accurately, comparing it to the physician’s order and O.R. schedule Ascertains all pre-operative (lab work, chest x-ray, EKG, H&P) work up, if applicable is in the chart and reports abnormal findings to the anesthesiologist and/or surgeon prior to the scheduled day of surgery Reports pertinent pre-operative information to the O.R. nurse Obtains and labels personal belongings bag and assists patient in proper O.R. dress attire Registration form and pre-operative worksheet Administers pre-operative sedation An order always precedes the administration of medication Allergies questioned prior to administration of medication Intended action and side effects to medications are known Medication is documented on Pre-Operative worksheet including does time, and method of administration Functions independently in emergency situations Displays proficiency in nursing skills with emergency situations Able to make logical and quick decisions based on sound judgment Maintain patient and employee privacy and confidential information provided psychological support to patients and/or families Maintains rapport with physicians and medical departments which provide services to complete to pre-operative work up i.e. (reference lab, outside radiology services) Maintains competence through continuing education Meets requirements for CEU re-licensure Attends all facility in-services Assists in orientation of new employees as directed Maintains supplies and cleanliness in the work environment Reports and labels any malfunctioning or broken equipment Works within the established facility and departmental safety guidelines Performs other duties as assigned KNOWLEDGE, SKILLS, AND ABILITIES : Knowledge of healthcare products, stock rotation procedures, storage systems, inventory supplements, preference card management and credit Knowledge of medical terminology as related to healthcare products Knowledge of inventory control procedures Hands-on, a self-starter with the ability to multi-task, be detailed oriented and possess superior organization and communication skills Must be able to handle a large volume of detailed work accurately Must have working knowledge of Microsoft Office products Must have superior leadership, communication and organizational skills Must be able to bend, stoop, lift, and handle all supplies Must have a strong ability to interact with co-workers, vendors, and management on a regular basis and in a pleasant and efficient manner Ability to perform simple mathematical functions Ability to perform data entry procedures and both equipment and supply tracking EDUCATION/EXPERIENCE : Graduate of an accredited school of nursing Current Registered Nurse License Current American Heart Assn.
