As part of the Department of Disabilities, Aging and Independent Living (DAIL) Adult Services Division, the Choices for Care program helps older Vermonters and people with disabilities receive long-term care services & supports in the setting of their choice. The Nurse Case Manager/Utilization Review Nurse I's role is to perform in- person/virtual clinical eligibility assessments and Utilization Review for Vermont's Long-Term Care Medicaid Choices for Care (CFC) Program, Brain Injury Program (BIP), Attendant Services Program (ASP) and for the Adult High Technology (AHT) Program. This position is based in Rutland and will include regular travel to neighboring counties. Work is performed under the general supervision of the Nurse Administrators and Program Manager. RN licensure is required, experience with case management preferred.
