Job Description Nemours Children's Health is seeking an experienced Pediatric Advanced Practice Nurse for our Dover, Delaware office! Extended hours are required (early morning or early evening). Occasional weekend coverage. At Nemours Children's Health, we see patients for primary care medical needs like well-child checkups, sick-child care and immunizations. Key Responsibilities Comprehensive patient assessments including history and physical examinations Formulating assessment and treatment plans of pediatric patients Communicating with physicians about changes in patient’s clinical condition Educating parents/caregivers on patient condition and plan of care Triaging and responding to phone calls from patient families Accurately recording patient information in the electronic medical record The NP must be able to work independently as well as part of multidisciplinary team. Responsibilities: Provide direct, professional nursing care to specified population of patients in the acute care setting. Develops a patient centered plan of care based on individualized needs. Maintains confidentiality of the patient medical record. Counsel and educate patients and their care partner on treatment plans. Administer medication following the five rights of medication administration. Provide wound care and dressing changes, as applicable. Read and interpret patient information, making treatment decisions, when appropriate. Responsibilities: Follows nursing assistant plan of care established by the Registered Nurse within a specified timeframe; including, but not limited to, mouth care, bathing, AM and PM care, assistance with ambulation, transfers to chair and/or bed, taking and documenting vital signs, and recording accurate intake and output. Documents all patient care delivered according to established policies and procedures. Performs duties with the knowledge and skills necessary to provide care appropriate to the age of patients served on the assigned units. Maintains a safe, comfortable, and therapeutic environment for patients and families in accordance with hospital standards. Communicate relevant patient information to a professional nurse within an appropriate time based on the level of urgency. Report problems immediately to a professional nurse. Answer intercom or call lights in a timely, courteous manner, with appropriate communication to the patient and/or professional nurse.
