Must have proven problem-solving abilities for routine situations and request assistance from identified resources when needed. 9. Must be able to read, write, and communicate in English in a clear and concise manner. 10. Must utilize proper safety techniques and body mechanics in all work related activities. 11. Must be able to remain calm when under stress or in emergency situations. Preferred: Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements. Brief summary of purpose: The Nurse Case Manager (NCM) is an integral part on an interdisciplinary team focused on care coordination, care management and improving access to and quality of care for Fallon members. NCM seeks to establish telephonic and/or face to face relationships with the member/caregiver(s) to better ensure ongoing service provision and care coordination, consistent with the member specific care plan developed by the NCM and Care Team. Lead HIV/Hepatitis and Nutrition groups and complete associated documentation. Attend all scheduled staff meetings. Maintain clear, written logs of client activities. Report significant questions, concerns, and complaints to counselors or administrative staff. Complete all required paperwork including medical logs, shift reports, and MORs. Maintain prompt and regular attendance. Assist in the medical training of BHTs as needed. Be available for overtime if required. Perform other duties as assigned by senior management. Qualifications/Required Experience Licensed Registered Nurse in the state of Massachusetts CPR Certification required. Several years of experience in a substance abuse or psychiatric setting. Strong verbal and written communication skills. Ability to work 12-hour shifts and must be available to work weekends and have schedule flexibility. Education & Licensure Registered Nurse (RN) credential in Massachusetts required. CPR certification required and must be maintained during employment.
