Create a therapeutic environment while promoting social skills and community engagement. Collaborate with a multidisciplinary team to implement individualized care plans. Maintain high standards of care, service excellence, and ethical practice. Key Responsibilities: Assist with daily living activities, including bathing, dressing, grooming, and feeding. Monitor and report changes in resident condition. Ensure safety and comfort by maintaining a clean and organized environment. Support residents with physical activities, such as ROM exercises and mobility assistance. Accurately document care provided and observations. Participate in training, quality improvement, and team collaboration. Qualifications Education: High School Diploma or GED required. Certification: Must be a Certified Nursing Assistant (CNA) or be willing to obtain certification within three months of hire. Experience: Experience working with individuals with severe disabilities is preferred but not required. Skills: Compassionate, patient, and dedicated to improving the lives of others. To implement support per the individual’s support plan, responding to multi-disciplinary team communication. Responsibilities Essential Functions Responsible for the overall physical, psychological and emotional health and safety of the participant.Models healthy behavior and positive social skills.Provides supports as written in the individual’s support plan that may include personal care, physical lifting and therapeutic holds.Provides feedback to team members and/or supervisors as to effectiveness of individual plans.Assists with an individual’s environment such as cleaning, laundry, shopping, cooking, errands and budgeting.Facilitates and maintains family contacts and community involvement.Encourages the development of new relationships and contacts by assisting people to be active in their communities and workplace.Communicates effectively in verbal and written format.
