Job Description The ECM Case Manager - Care Coordinator is responsible for assessing the strengths and needs of clients enrolled in the Permanent Supportive Housing (PSH) program. This role includes linking clients with appropriate resources, developing individualized housing and service plans, and monitoring progress toward the goals established in those plans. The ECM Case Manager empowers clients on their path to self-sufficiency by providing trauma-informed care, practicing active listening, and supporting client engagement. They play a key role in connecting clients to essential services, including medical and mental health care, substance use treatment, financial assistance, and other supportive programs. In addition, the ECM Case Manager is responsible for documenting client progress and maintaining accurate records in alignment with organizational standards. This position requires the ability to maintain strict confidentiality and handle sensitive information with professionalism, tact, and diplomacy. The pay range for this position is $24.00 per hour The schedule for this position is Monday - Friday, 9:00am to 5:00pm Responsibilities Maintain a caseload of up to 35+ individuals, providing consistent and individualized case management support.
