Position Summary

Responsible for the delivery of patient care that promotes safety and well being of all patients in the assigned department. Plans, directs, coordinates and evaluates the overall clinical nursing practice, daily functioning and management of the Long Term Care Facility Nursing Department Maintains open communication with department staff to facilitate performance improvement. Position Qualifications:

  • Must have completed a program of education in nursing from an NLN approved institution. BSN Preferred
  • 2 years charge experience in a Clinic or Urgent Care facility
  • In addition, must be licensed in good standing in the State of Alaska as a Registered Professional Nurse. Current BLS. The successful candidate will be required to pass and maintain competencies for this position.
  • Office equipment (i.e. computer, telephone, fax machine, copy machine)
  • Medical equipment such as dynamap, scales, thermometer, etc.
  • Proficient oral and written communication skills.
  • Must be licensed in the State of Alaska

Registered Nurse Care Manager - Clinical Diabetes Educator; $30,000 Sign On, $15,000 Relocation, and Retention Bonus eligible

Position Summary : This is a grant funded position. Registered Nurse Case Manager/Clinical Diabetes Educator is primarily responsible for providing comprehensive diabetes education and care for patients with Type 1 diabetes, Type 2 diabetes, gestational diabetes, and patients at high risk of developing diabetes in a clinic setting. Education is provided via telephonic communication, video teleconferencing, and in-person education sessions. This at times does require village travel. Educator will be responsible for ordering labs for patients according to hospital policies. Educator will be responsible for documenting education in electronic health record. This position will assist in the planning, coordinating and providing of diabetes-related health promotion activities in the YK Delta Region. Patients and activities are accessed via multiple village trips per month. Educator will work closely with village clinic and hospital staff to promote diabetes standards of care and inform of new advances in diabetes care. Educator assists Diabetes Coordinator with maintenance of the Diabetes Registry and grant requirements. Educator works closely with the Registered Dietitian Clinical Diabetes Educator and Diabetes Pharmacist to ensure comprehensive care of clients. Educator participates in all program outreach activities such as monthly Diabetes Support Groups and radio shows, media campaigns, and special events.
