Current BLS, NRP, & Fetal Monitoring.

MAT Clinical Nurse Care Manager - RN

Position Summary: This position is accountable for assessing, planning, coordinating, monitoring and evaluating services for patients and their families, thus fostering quality, continuity and appropriate utilization of health care resources throughout the continuum of care. The Clinical Nurse Care Manager will help coordinate the various aspects of the Medication Assisted Therapy (MAT) program and act as a bridge between the Behavioral Health and Medical components of the treatment program. The role combines direct patient care in combination with all required details of the care management of patients who utilize the MAT program for the treatment of substance use disorder. The Clinical Nurse Care Manager will collaborate with MAT medical providers, MAT pharmacists, MAT behavioral health clinicians and counselors, and the staff of YKHC scheduling, YKHC travel, and village services to coordinate the timely and effective care of both current and prospective patients utilizing the MAT program to promote the best outcomes.
