Perioperative Care Navigator

Advanced Practice Provider - NP/PA

Per Diem

The Perioperative Care Navigator at Hospital for Special Surgery (HSS) will coordinate, monitor, and evaluate patients’ clinical needs as they prepare for surgery. The Perioperative Care Navigator is committed to working collaboratively and serving patients with exceptional care and promoting the mission, vision, and core values of Hospital for Special Surgery. Reporting to the Vice President of Inpatient Nursing, the Perioperative Care Navigator will support perioperative teams across multiple service lines. Given the nature of this role, this position will have a hybrid work environment, including telehealth and in person connections with patients and the interdisciplinary team.

Key Responsibilities:

Patient Management

Liaise with patients throughout their treatment journey, from surgical booking to surgery.
Conduct patient outreach calls to assess individual needs and identify specialized pre-operative referrals.
Provide education and support to patients in connection with their treatment/discharge plans.

Clinical Team Coordination

Gather and evaluate information from various clinical teams to determine appropriate patient discharge disposition.
Collaborate with interdisciplinary teams to coordinate scheduling surgeries and appointments, optimize patient experience, and coordinate post-discharge care.

Administrative Management

Adapt workflow according to patient care needs.
Maintain standardized care pathways or protocols to provide guidance for the delivery of perioperative care and streamline processes.
Strong organizational and problem-solving skills, with ability to derive practical solutions independently in a fast-paced environment, escalating issues appropriately when necessary.
Monitor care management processes.

Qualifications

Education and Experience:

For Nurse Practitioners:

Master’s degree from an accredited school of nursing.
FNP required
Current NYS License as a Nurse Practitioner and RN.
Board certification as a Nurse Practitioner.
BLS

For Physician Assistants:

Bachelors degree required, Master in PA studies preferred
Current NYS LIcense as a Physician Assistant
Infection Control
Child Abuse Mandate
BLS

Competencies and Skills:

Demonstrates exemplary judgement
Demonstrates interpersonal communication and team based skills
Demonstrates effectiveness and accuracy in the completion of responsibilities
Demonstrates ability to work in team environment
Demonstrates the ability to interact directly with patients in such a way to instill confidence and facilitate communication

All candidates:

Epic EMR experience
Proficiency with Office 365
