The best women’s hospital in Los Angeles is looking for current RN candidates to staff Operating Room, Emergency Department, Labor & Delivery and Mother-Baby care units to be housed in our expanding new patient tower facility scheduled to open next year. Position Summary:

Providing exceptional patient care and ensuring a smooth transition for patients who are ready to be discharged from the hospital. Reports to the Administrative Nursing Supervisor day to day duties.

Duties and Responsibilities:

Patient Assessment: Conduct comprehensive review of the completed discharge assessments by the registered nurse of patients' physical and emotional well-being to determine their readiness for discharge.
Care Planning: Collaborate with the healthcare team to develop individualized care plans that address the specific needs of each patient during the discharge process.
Medication Administration: There will be no indication to Administer medications in the discharge lounge. Prescriptions can be delivered by the pharmacy while patient awaits transport.
Patient Education: Monitor patients for safety and explain with clear instructions the waiting process.
5. Communication: Maintain open and effective communication with patients, families, physicians, and other healthcare professionals to ensure a coordinated and efficient discharge process.
Resource Coordination: Collaborate with social workers, case managers, and other healthcare professionals to arrange for necessary resources and services, such transportation.
Safety and Infection Control: Adhere to all safety protocols and infection control measures to ensure the well-being of patients and staff members.
Continuous Improvement: Participate in ongoing professional development activities to enhance knowledge and skills related to discharge planning and patient care.

Minimum Education:

High School Diploma

Graduate of an accredited LVN program

Preferred Education:

Minimum Work Experience and Qualifications (Indicate minimum years of job experience, skills or abilities required for the job.)

Minimum one year recent experience in an acute care setting desired, preferably in Emergency Department.

Ability to communicate effectively in English, both verbally and in writing.
Excellent communication and interpersonal skills, with the ability to provide emotional support to patients and their families.
Ability to work effectively in a fast-paced and high-stress environment, maintaining composure and professionalism.
Strong organizational skills, attention to detail, and ability to prioritize tasks.
Must be able to work in a union environment.
Ability to communicate effectively verbally and in writing.
