LPN/LVN - Licensed Practical Nurse/Licensed Vocational Nurse

POSITION SUMMARY

The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

HIRING REQUIREMENTS

Must be a licensed vocational/practical nurse, licensed in the state in which he/she practices.
Six months to one year of experience as a licensed nurse preferred.
One-year acute care preferred; 6 months pediatric experience in an acute or home health setting is preferred, or preceptorship as required by nursing knowledge.
Possess and maintain valid CPR certification.
Complies with accepted professional standards and practice.
Demonstrates good verbal and written communication, and organization skills.

ESSENTIAL RESPONSIBILITIES

Demonstrates a working knowledge base of the child’s unique response to illness and provides an environment that will foster optimum growth and development.
Teaches and assists the family in functioning independently in the care of their child.
Provides excellent technical skills during direct patient care. Consults with other disciplines as required and under the direction of the RN, adjusts nursing care processes as necessary to assure optimal patient care.
Provides care in accordance with the Nursing Care Plan. Provides feedback to the clinical director/supervisor regarding the assessment of patient’s needs. Plan of Care is developed by the Clinical Director/Supervisor.
Reevaluates the patient’s needs with respect to physical assessment, the plan of care and the patient’s response to same. Notifies the Clinical Director/Supervisor with any changes in the patient’s condition or response to treatment.
Demonstrates respect for the rights of the patient and family and respects the confidentiality of any and all information relating to the patient.
Insures an adequate stock of supplies and proper functioning of equipment.
Completes all documentation requirements in accordance with Thrive SPC's policy.
Provides patient updates to the Clinical Director/Supervisor on all clients in a regular and timely manner.
Maintains and enhances professional knowledge base by participation in staff meetings, conferences, inservices, and professional organizations as required by state/location.
Supports the goals of the Quality Department.
Acts as a patient advocate.
Acts as a representative of Thrive SPC by supporting and adhering to departmental policies, procedures, objectives, quality improvement programs, safety, environmental and infection control standards (including but not limited to HIPPA, OSHA, State & federal Standards and / or Regulations).
The Licensed Nurse may NOT admit patients to the Thrive SPC or discharge patients from Thrive SPC.
Other duties as assigned.

KNOWLEDGE AND SKILLS

Must possess effective communication skills and the ability to respond to the needs of clients and caregivers in the home setting.
Must have excellent assessment skills.
Has a thorough understanding of home health regulations.

TRAVEL REQUIREMENTS

Travel to client homes and office locations

ESSENTIAL PHYSICAL DEMANDS

Risk Category: I

May anticipate exposure to blood and/or body fluids during the performance of job duties.
May be exposed to extremes in temperature and humidity when traveling and in client homes.
May be exposed to dust, gas, fumes, and odors when traveling.

Physical Demands:

Travel to other branch offices, parent office and/or client's homes
Manual dexterity to operate patient care equipment.
Operate office equipment including, but not limited to, computers, printers and/or copiers.
Visual acuity to evaluate and peruse employee documentation.
Frequently required to lift/push/carry up to 50 pounds.
