Community Health Registered Nurse

Sidney Regional Medical Center is hiring a Community Health Registered Nurse. The Community Health RN will partner closely with our current Community Health Coordinator to expand current services to both the community and internal SRMC departments. This is a Monday through Friday 8am-5pm opportunity, with potential for varied shifts based on community events and needs.

Job Responsibilities:

Work with local businesses to set up and coordinate various appointments for pre-employment testing such as physicals, hearing screens, laboratory tests/drug screens and ensures completion in a timely manner with results faxed to appropriate employer contact.
Organize and implement annual Sidney Regional Medical Center Health Fair, Influenza vaccines for businesses as well as community flu clinics. Administers Hepatitis B vaccines, tuberculin and Tdap upon request per businesses for their employees.
Gather statistical information and record keeping.
Organizes and implements blood pressure clinics in geographical areas, as well as for businesses upon request.
Responsible for coordinating the diabetes education program along with the Registered Dietitian and providing diabetes education to patients being referred by their provider to the program. Will oversee the planning, implementation and evaluation of the diabetes self-management education entity.
Responsible for developing implementing and monitoring policies and Procedures related to Diabetes Education
Responsible for coordinating Community Blood Drives up to five times a year.
Responsible for coordinating Annual Health Fair.
Works with community corporations to assist with components of their individual wellness programs.
Responsible for setting up and co-facilitating the Diabetes Education Advisory Group.
Responsible for providing diabetes education to the community.
Serves as the RN back-up for Employee Health
Other duties as assigned.

Knowledge, Skills, and Abilities:

Ability to prioritize workload and adapt to changing work assignments and/or responsibilities
Positive, TEAM-oriented attitude
Strong communication skills, with ability to partner with all clinic leaders at all levels
Remain sufficiently knowledgeable about the legal aspects of his or her responsibilities and activities to be able to avoid inadvertent violation of State and Federal statutes and regulations.
Expected to comply with applicable laws related to their job responsibilities and refrain from knowingly participate in illegal activities or failing to meet affirmative legal duties.

Education & Experience:

Graduate of an accredited RN school of nursing and hold a current license in the State of Nebraska as a Registered Professional Nurse.
Achieve Certified Diabetes Educator status when all requirements met.
6 months to one year of related experience and/or training or equivalent combination of education and experience.
Must have and provide a current and valid driver’s license. Provide proof of personal auto insurance with coverage of $100,000.
Must have academic preparation and/or experiential preparation in program management and in the care of persons with chronic diseases.
