Schedule is Monday-Friday days + call responsibilities.

Current call responsibilities include overnight on call by phone approximately once every other week, availability for weekend death certificate completion, if needed, as part of a rotating schedule, and one holiday per year.

A Nurse Practitioner serves as an advanced practice clinician providing primary level of care in collaboration with physicians and other members of the health care team.

Essential Duties and Responsibilities of a Nurse Practitioner:

Effectively identifies, evaluates, and addresses disease progression and symptom management, under the direction of a physician.
Works in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration, or referral.
Responsible for the management of acute and chronic conditions related to the primary diagnosis identified for hospice eligibility.
Provides palliative medicine consults for symptoms and psychosocial needs associated with chronic, serious illness, as appropriate
Educates patients and families about disease progression and treatments.
Performs physical examinations and health measures within prescribed guidelines and instructions of physicians.
Orders, interprets and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.
Records physical findings, and formulates plan and prognosis, based on patient's condition.
Discusses case with physician and other health professionals to prepare comprehensive patient care plan.
May prescribe or recommend drugs (as allowed by license) or other forms of treatment, as necessary.
Facilitates evaluation of records by IDG and QI according to protocols and receives and implements constructive directives.
Participates in the provider call schedule and cross-coverage responsibilities.

Qualifications for a Nurse Practitioner:

Education/Certification
Graduate of an accredited school with current NP licensure in the State of New York.
Board Certification
Valid Driver's License, free of serious violations, and vehicle required for use when on duty or on-call.

Experience
- Minimum three (3) years of clinical nursing experience required.
- Prior acute care experience preferred
- Hospice and palliative care experience helpful
