Qualifications:

  • Physician Assistant: Medical/Master’s degree with a valid state license and national certification.

  • Nurse Practitioner: RN license, NP certification, and a valid state license.

Requirements:

  • Valid, unrestricted medical and DEA licenses.

  • Ability to treat patients of all ages.

  • DOT Certification: Must be obtained prior to your start date. Carbon will cover the course and exam fees and provide a roadmap and resources to help you succeed.

  • BLS and/or ACLS certification is required and must be obtained before your first day with Carbon Health.

Nurse Practitioner (NP) - COACH for Kids Mobile Clinic - Per Diem - 8hr Days

The Cedars-Sinai mentor program, COACH (Community Outreach Assistance for Children’s Health) for Kids, is a mobile medical clinic. It centers on addressing the immediate medical, behavioral health, and social needs of underserved children in South Los Angeles. With a community-centered strategy, the program provides free, temporary care and connects families to continuous, quality healthcare and essential community resources. Mentor for Kids provides services directly within the community via a brand new, fully equipped, wheelchair-accessible mobile medical clinic. The schedule typically covers Monday through Thursday from 9:00 a.m. to 3:00 p.m., with occasional weekend engagements. Care takes place at accessible community venues such as schools, places of love, public housing developments, grocery stores, WIC (Women, Infants, and Children) centers, Head Start programs, homeless shelters, and other neighborhood-based organizations throughout Central and South Los Angeles, including Watts and Crenshaw. A core goal of the program is to ensure continuity of care. It connects patients to Federally Qualified Health Centers (FQHCs) and other medical homes. These providers support the long-term health needs of the entire family. COACH for Kids reduces barriers to care and improves health outcomes by meeting families where they are and supporting their transition into comprehensive, continuous healthcare systems. Responsibilities: Provide comprehensive primary care to pediatric patients in community settings via a mobile clinic, including well-child visits and preventive services. Conduct thorough histories, physical exams, and assessments; develop individualized care plans. Diagnose/manage acute and chronic conditions; perform diagnostic and therapeutic procedures. Administer immunizations, screenings, and point-of-care testing; interpret results and follow up. Educate patients and families on health promotion and disease prevention, tailored to their needs. Collaborate with healthcare team and community partners for coordinated, high-quality care. Facilitate referrals and coordinate with FQHCs, specialty care, and community resources. Serve as onsite clinical lead, supervising staff and ensuring efficient mobile clinic operations. Document patient care accurately in electronic medical records. These services often take place in hospital or rehab environments. They include medication management, therapy, and interventional procedures. Job Description What will you be doing: The Neonatology Nurse Practitioner (NP) Per Diem is responsible and accountable for the application of the nursing process, supervision of clinical practice, research involvement, consultative services, inter and intradepartmental liaison, education, leadership, and patient care management across the health care continuum from preventative, acute care through discharge. The Transplant Coordinator III-NP, under the direction of a collaborating physician, assists in the care of patients with a variety of end stage organ diseases. Coordinates and participates in the medical/surgical management in either inpatient and/or outpatient setting of transplant and/or end stage organ diseases. Performs comprehensive history and physical exams, develop differential diagnoses, implement plan of care and document patient's responses. Order medications, diagnostic studies, and therapeutic procedures related to their illnesses and track results timely as outlined. The Transplant Coordinator III-NP also assists other personnel to ensure high quality delivery of patient care and services to a transplant/end stage organ disease patient population. Develops and coordinates a system to organize the flow of patients from initial referral, through evaluation and education, to post-transplant follow-up and referral back to the community. Participates in collaborative, interdisciplinary research activities related to transplant patients, process improvement or evidenced based practice activities. Attend professional meetings and conferences, maintain professional affiliations and enhance professional development to maintain knowledge of advances of transplant populations and end-stage organ diseases.
