Job Summary
Performs the activities within the Prisma Health Referral Center to facilitate referral volume from outside health care facilities and physician offices. Accomplishes the responsibilities of this position by integrating and maximizing the work of various disciplines, resources and services across departmental lines. Coordinates patient admissions and transfers within facility bed availability and scope of service. Evaluates admission/transfer process for patients, families, and physicians on an individual and collective basis.Essential Functions

Facilitates referral volume from outside health care facilities and physician offices. Accomplishes the responsibilities of this position by integrating and maximizing the work of various disciplines, resources and services across departmental lines.
Maintains Quality and Compliance for the Prisma Health Referral Center.
Collaborates, as appropriate, with other departments and entities to ensure efficient patient throughput.
Operates within the established budget set forth for the Prisma Health Referral Center. Coordinates patient admissions, transfers and bed availability on specified clinical areas.
Performs within the bed capacity of every facility within Prisma Health. Assures that patient movement is maximized so that lack of room for a referred patient is a non-issue. Works very closely with Central Bed Management.
Conforms to and takes the initiative to be aware of all general rules, regulations, policies and practices of the department and system.
Attends the appropriate meetings and communicates needs to other disciplines along with known or anticipated problems.
Completes post-discharge phone calls as needed.
Responds to complaints by listening and taking appropriate corrective action in a timely manner. Refers problems to appropriate individuals if unable to resolve.
Performs other duties as assigned.
Supervisory/Management Responsibilities
This is a non-management job that will report to a supervisor, director or executive.
Minimum Requirements
Education - Associate degree in Nursing
Experience - Three (3) years Acute Care Nursing. Nursing Supervision experience preferred

Required Certifications, Registrations, Licenses
Holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working.
