Job Summary:

The Primary Care Advanced Practice Clinician (APC) is a Nurse Practitioner (NP) or Physician Assistant (PA) who works in collaboration with a multidisciplinary health care team. The APC provides healthcare in primary, urgent, community-based, and long-term care settings, delivering services to individuals and families across the lifespan. The APC has didactic education, clinical competency, and national certification in the identified area of clinical practice, enabling the delivery of preventive healthcare as well as the assessment, diagnosis, and treatment of acute and chronic illness.

The APC demonstrates an ongoing commitment to patient-centered care and partners with individuals and families to promote engagement and shared decision-making, supporting optimal health, well-being, and self-management of disease processes. Supported by advanced knowledge of clinical theory and its application, the APC demonstrates evidence-based medical knowledge, clinical judgment, technical competence, professionalism, effective interpersonal communication skills, timely and compliant documentation, and lifelong professional development and competency validation.

Position Hours:

  • Days - Monday through Friday
  • 4.5 days per week
  • Hours are generally 7:30/8:00 to 4:30/5:00
  • No Weekends or Holidays
  • Call Required

Requirements:

  • Prior APP Experienced Required
  • FNPs, ANPs, Adult-Gero Primary Care NPs, and PAs
  • Experience: Minimum of 2 years in primary care, urgent care, occupational health, or student health clinic) strongly preferred

KEY RESPONSIBILITIES

  • Obtains and documents a health history; performs and documents complete, system-focused, or symptom-specific physical examination, assessment and plan of care.
  • In collaboration, provides healthcare services for primary, acute and complex care of patients and manages patient's overall care, identifying expected outcomes for diagnoses.
  • Orders, performs, interprets and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated.
  • Forms differential diagnoses and treats acute and chronic conditions.
  • Prescribes therapeutic interventions both pharmacologic, non-pharmacologic and surgical, needed to achieve expected outcomes.
  • Utilizes evidence-based, approved practice protocols in planning and implementing care;
  • Initiates appropriate referrals and consultations;
  • Provides specialty specific consultation services upon request and within specialty scope of practice;
  • Facilitates the patient’s transition between and within health care settings, such as admitting, transferring, and discharging patients.
