Role Overview We’re seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across Stockton, Modesto, and Amador. What You’ll Do Routine & urgent assessments; chronic disease management Post-discharge follow-ups, med reconciliation, labs/diagnostics Coordinate with facility staff, specialists, families/POAs Same-day documentation; CMS/regulatory compliance Schedule & Setting Flexible weekday schedule. Full-time preferred, with part-time options available. Providers deliver on-site care across Stockton, Modesto, and Amador with strategically clustered routes designed to minimize travel and maximize patient care time.
