Compensation: $90,000+ per year depending on licensure and experience

Position Overview
The Wellness Director (RN/LPN) oversees all clinical services within the community and ensures the delivery of safe, compliant, and resident-centered care. This role combines clinical leadership, nursing oversight, staff supervision, and regulatory compliance to support the health and well-being of residents.
Because Legacy Village of Salt River is a large, 200+ bed community, the Wellness Director plays a key role in building strong systems, improving clinical workflows, and ensuring consistent care across a large team.
We are looking for someone who is creative, solutions-oriented, and open to trying new processes or improving existing systems to strengthen care delivery and team efficiency. The right candidate enjoys identifying opportunities for improvement and implementing thoughtful changes that benefit both residents and staff.
Key Clinical Responsibilities
- Coordinate comprehensive resident assessments, including initial, routine, and change-of-condition evaluations
- Develop, implement, and update individualized service plans and care plans
- Oversee medication management systems, ensuring accurate administration and documentation
- Monitor residents for changes in health status and coordinate care with physicians, pharmacies, and families
- Maintain clinical documentation and regulatory compliance according to state and company standards
- Provide hands-on clinical support and guidance to caregiving staff when needed
- Supervise, coach, and mentor care staff and medication technicians
- Lead infection control practices and safety protocols
- Manage incident reporting, follow-up, and quality assurance initiatives
- Coordinate care with outside healthcare providers, therapy partners, and hospice services
- Participate in care conferences and family communication
- Evaluate current workflows and implement process improvements that enhance resident care and staff efficiency
Schedule
This is a full-time leadership position with a consistent schedule. The Wellness Director participates in on-call support to address clinical needs or urgent situations outside regular business hours.
Qualifications
- Active RN or LPN license in the state of Arizona
- Experience in assisted living, memory care, or senior living preferred
- Strong background in resident assessments, medication management, and care planning
- Knowledge of state regulations and compliance requirements for assisted living communities
- Experience leading clinical teams in a larger community or multi-unit environment is preferred
- Ability to think strategically and creatively when improving processes or workflows
- Strong leadership and staff development skills
- Excellent communication skills with residents, families, and healthcare providers
