Position Description:

 

The Case Manager, a Licensed Vocational Nurse, under the Director of Case Management, serves as an interdisciplinary team member to coordinate the delivery of health care services leading to quality outcomes for the inpatient hospital population and their families with the limits of the financial and social resources available to the patient and his family. This person is an expert on issues related to utilization review and discharge planning. 


Qualifications

Job Duties/Responsibilities Requirements:

  • Minimum five years of acute care clinical experience and minimum of three years Clinical Case
    Management/Utilization Review experience (preferred). Motivation for self-direction.
  • Minimum of LVN program.
  • Written and verbal communications, interpersonal skills.
  • Basic organizational and computer skills.
  • Current LVN California license, current BLS.
