Job Summary
Performs Home visits for trauma patients post-discharge. Participates in community education on injury prevention and trauma recovery. Coordinates with local health agencies, shelters, and social services. Performs mental health screening and referrals. Provides support for victims of violence and domestic abuse. Provides follow-up care for trauma patients. Wound care, medication management, and rehabilitation coordination. Participates in multidisciplinary team collaboration, Trauma Clinic, and Trauma Quality of Life Clinic. Ensures documentation and data collection for quality improvement.
Education

Education
- Bachelors Degree : in Nursing from an accredited school of nursing. (Required)
- Masters Degree : in Nursing from an accredited Nursing program. (Required)
- CRNP - PA Cert Reg Nurse Practitioner (Required)
- RN-LIC - PA Registered Nurse License (Required)
- PRX-AUTH - Prescriptive Authority (Preferred)
- BLS - Basic Life Support (Required)
- 2 Years experience as a nurse practitioner (Preferred)
- 1 Year experience in a clinical specialty practice (Preferred)
- 1 year outpatient/ambulatory experience (Preferred)
- Experience in an EPIC environment (Preferred)
