Overview:
Utilizes the nursing process to provide patient care. Maintains professional accountability for provision of patient care for the assigned patients. Evaluates the overall effectiveness of care provided by other direct care givers. Coordinates the patient care in conjunction with other departments. Maintains patient rights and confidentiality of patient information. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.

Qualifications:
Education: Graduated from a school of nursing (Associate's Degree, Diploma, or BSN).
Licensure: Is personally responsible for obtaining, and maintaining, a current RN license within the hiring state and/or compact licensure in which nursing duties are performed and must meet all state board of nursing requirements.

Certifications:
- Basic Life Support certification through the American Heart Association or successful completion of course within 30 days of hire.
One or more of the certifications below may be required based on the position/unit hired to, or acquisition of certification within department required timeframe:

The Intake Clinician responds to inquiry calls, conducts initial intake assessments, provide referrals, and coordinate admissions process according to Joint Commission, Federal and State Regulations, Oceans' Mission, policies and procedures and Performance Improvement Standards. Essential Functions: Responds to inquiry calls, completes inquiry forms accurately, schedules assessments in a timely manner and documents follow up calls made regarding pending assessments. Conducts assessments at facility or other locations, completes Comprehensive Interdisciplinary Assessments Initial Triage thoroughly. Refers patients who are not appropriate for facility programs to appropriate resources. Coordinates pre-certification process, admissions process for patients admitted to facility programs, including admission forms, CIAs, and documents pertinent UR information. Delivers patient services in a respectful manner which promotes dignity and patient satisfaction. Identifies safety needs including ability to detect possible signs of abuse and neglect and report same to clinical staff. Identifies special mobility and equipment needs of patients Assess and interprets age and disability needs to assess level of care needed and made appropriate referrals based on same. Communicates with patients and families in manner sensitive to their age, disability and cultural needs and involves families of minors or significant others in decision making related to treatment as appropriate. Performs other duties and projects as assigned.
