This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community sources. The Registered Nurse (RN) will work in either a Case Management, Admissions, or Resource role. Qualifications Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership. Work closely with patients on a 1-1 level but get strong support from a patient care team. Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations. Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns) Ability to learn an electronic medical records system. Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance, to be able to travel across the service area

As a RN, you will complete hospice admissions and patient visits as needed. Applicants must have great clinical and communication skills. The RN is responsible for answering calls from patients, families and customers. The RN will make nursing visits for symptom management, deaths, and admissions as needed. This RN must also be passionate about hospice care and be a great advocate for both hospice services and Suncrest Hospice. This position will be after hours on-call working seven days/nights on and seven nights/days off. Qualifications Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice with one year of experience under license Admissions and On-call experience preferred Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership. Records physical findings, and formulates plan and prognosis, based on patient's condition. Discusses case with physician and other health professionals to prepare comprehensive patient care plan. Prescribes or recommends drugs or other forms of treatment. Monitors and maintains progress of patients and reports progress to supervising physician. Has the ability to work collaboratively with hospice employees and volunteers as part of an IDT Qualifications While performing the duties of this position, the employee is regularly required to assist in lifting and transferring patients as required. Specific vision abilities required by this job include: close vision, distance vision and ability to adjust focus. Requires completion of accredited Nurse Practitioner program.
