Responsibilities

Schedule: Mon-Sun, 14/12 hour shifts per 28 days. Shift rotation of 3 months day shift and 3 months night shift. On-Call: 2 days of call per 28 day cycle with the possibility of working up to the full 12 hours if called in. 6:45 a.m. - 7:15 p.m. (Day Shift) or 6:45 p.m. - 7:15 a.m. (Night Shift)

The duties include but are not limited to the following:

Facilitate the achievement of optimal outcomes in relation to clinical care, quality and cost effectiveness.
Rotate between day and night shifts on either of the Medical Surgical Units in David Grant Medical Center (4W or 3C).
Document patient care accurately into Electronic Health Record of all patient care activity in accordance with facility requirements.
Work under the leadership of the RN staff, shift leaders, charge nurses, and the unit Flight Commander.
Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF.
Perform physical exam and health histories under RN guidance.
Recognize adverse signs and symptoms and quickly react in emergency situations.
Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions.
Ability to synthesize data from a variety of sources and make appropriate clinical decisions.
Skilled in setting up, operating, and monitoring specialized medical equipment such as cardiac monitoring devices, vital signs, EKG units, oxygen analyzers, nebulizers, and other unit specific equipment.
Assists with patient admissions, discharges, unit specific medication administration, and other duties within the scope of practice of the LPVN.
Participates in the orientation and training of newly assigned personnel, as appropriate.
Provides relevant and timely information to these groups and assists with decision-making and process improvement. Participates in customer service initiatives, and medical readiness activities, designed to enhance health services.
Ensures a safe work environment, employee safe work habits and patient safety IAW regulatory agencies, infection control policies, and process improvement initiatives.
Maintains current mandatory training and annual training updates that are required of members on the unit.
Report any problems or issues on the unit to the Flight Commander.
Completes any assignments from the unit management within given time frames, adapts to schedule changes and changes to work assignments when necessary.

Qualifications

Minimum Qualifications:* Education: Licensed Practical/Licensed Vocational Nurse degree, completion of high school. Graduate from an accredited professional nursing educational program.

* Experience: Two years of experience in health care as identified in the TO after graduation.

* Licensure/Registration:Current, full, active and unrestricted license as a practical or vocational nurse.

* Life Support Certification: Possess a current American Heart Association BLS Healthcare Provider certification.

* Security: Must possess ability to pass a Government background check/security clearance. Pay Scale: $31.00 - $45.00 per hour, to be determined based on qualifications, experience, and location.

Make careful observations to assess that nursing procedures and treatment do not cause additional distress. Perform routine nursing care activities such as taking blood pressures, temperatures, passing and removal of trays, changing of linen, and otherwise assist in the care of the physical needs of the patient. Operate basic equipment required in delivery of patient care such as pumps, IV pumps, oxygen administration apparatus. Execute physician’s orders within the guidelines of standard nursing practice. Ensure accurate medication is administered in correct form and dosage to the proper patient as directed by the physician. Maintain records of nursing care, dose and time of medication administered, and indicate if the medication was not administered and the reason. Recognize conditions which require isolation. Ensure universal precautions are used in all patient encounters. Recognize emergency situations and assist with, or institute emergency measures for sudden adverse developments in patients such as cardiac arrests.  Perform preoperative procedures for minor surgery and fill out pre-op checklist. Assist patients in admission, transfer, and perform discharge planning follow-up and documentation. Support the patient and/or family members toward the achievement of treatment plan goals. Provide instructions to the patient on invasive procedures, surgical procedures and post-surgical conditions which were previously provided to the patient by a nurse or physician. Instruct patients on how equipment is used such as oxygen, suction, cardiac monitor, and pulse oximeter. Instruct patients and family on use of prescribed medications, contraindications of medications, and the necessity of proper follow-up care. Provide emotional support to patients and families. Ensure necessary supplies are available and equipment is in functioning order. Provide an orderly, clean, and safe environment for patients and staff. Handle telephone information requests with courtesy, accuracy, and respect for patient confidentiality. Receive information and distribute messages as necessary. Ensure maintenance and re-supply of pharmaceuticals in the clinic. Ensure upkeep and perform checks of emergency equipment i.e., oxygen, emergency cart, suction apparatus, etc. and maintain appropriate logs. Inform the Charge Nurse of any discrepancy on daily checks. Provide care coordination using specific criteria and guidelines as mandated by DOD, BUMED, DHA and TRICARE directives, coordinating with the local MHP clinic, utilization, and Referral Management policies, and obtains additional clinical data as required. Consult, coordinate, and collaborate with MHP team members, referral management team members, specialty care clinics, outside care professionals, agencies, organizations, and other ancillary support systems to assist in the care coordination of MHP patients. Identifies patients in need of care coordination within the MHP teams, through screening patients with a pattern of high risk, unstable and resource intensive utilization of Military Treatment Facility (MTF) or network services such as chronic pain patients. Responsible for coordination of outpatient care services of patients belonging to the MHP clinic area of responsibility that are discharged from other facilities. Follows-up with MHP team members regarding review of patient charts, medical records, and other documents utilized in the treatment and care of MHP patients in need of care coordination. Participate in Virtual health encounters as needed. Qualifications Minimum Qualifications: * Education: Be a graduate from a Licensed Practical Nurse or Licensed Vocational Nurse program accredited by the Accreditation Commission for Education in Nursing (ACEN) or be a graduate from an accredited program accepted by the applicable State Board of Nursing or a training program accepted by the MTF. * Experience: Possess six months of clinical work experience as a full-time LPN/LVN within the preceding four years. * Licensure: Current, full, active, and unrestricted LPN/LVN license from any state. Prior to commencing services, Licensed Practical (Vocational) nurses shall attend a 2-hour lecture on IV certification. Contractor nurses shall then obtain certifications (e.g., IV, medications and blood administration) in accordance with clinic nursing service and command policy not later than 60 days after commencement of services, including MTF observation of three completed IV sticks. * Life Support Certifications: AHA or ARC BLS Certification * Security: Must possess ability to pass a Government background check/security clearance. The salary range for this position is $29.81/hr. - $43/hr. Shifts will be 12 hours with a 30-minute lunch break. LPNs will rotate every four (4) months from shifts; however, schedules may vary to accommodate demand. Duty hours will be scheduled by the Labor and Delivery Flight Commander, or designee, to correspond with the needs of the department. Shifts times are Days (0545 – 1815) and Nights (1745 – 0615). Job Specific Details and Tasks: • Coordinate patient care in collaboration with a wide array of healthcare professionals. Facilitate the achievement of optimal outcomes in relation to clinical care, quality and cost effectiveness.• Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF.• Perform physical exam and health histories.• Provide health promotions, counseling, and education. • Administer medications, wound care, and numerous other personalized interventions.• Accountable for making patient care assignments based on the scope of practice and skill level of assigned personnel.• Recognize adverse signs and symptoms and quickly react in emergency situations.• Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization.• Shall assess patients, including assess/evaluate need telephonically and recognize adverse signs and symptoms and quickly react in emergency situations.• Shall collect and assess significant patient history information and perform all necessary patient teaching.• Shall communicate patient/family education and telephone triage of patients.• Shall provide timely responses to telephone and in-person assessment/evaluation include the determination of optimal time and location for patient management (ER, clinic, homecare) and follow-up care as required.• Shall perform/assist with procedures such as injections, IV medications/infusions, administration of oxygen, visual acuity tests, EEGs, EKGs, phlebotomy and other clinic specific procedures, in accordance with established protocols. Shall make referral appointments and arrange specialty care as appropriate.• Shall demonstrate skill in setting up, operating, and monitoring specialized medical equipment such as cardiac monitoring devices, EKG units, oxygen analyzers, nebulizers, and other clinic specific equipment.• Shall prescribe and communicate treatment plans and patient teaching in accordance with established protocols.• Shall address patient expectations and requirements, establishes level of need and priority of care in accordance with established policies and procedures.• Shall coordinate patient care with community health agencies and social programs. Shall facilitate continuity of care for patients. Has knowledge of a wide variety of resource agencies available in the military treatment system to ensure proper and expedient referral of the patient and/or family member.• Shall clean and store equipment properly at the end of each work shift. Shall ensure preventive maintenance and routine cleaning are performed as directed. • Shall perform administrative duties that may include maintaining statistical records of his or her clinical workload, participating in medical educations programs, and participating in quality improvement functions and other duties as prescribed by the Flight Commander.• May be asked to participate/direct committees, programs or clinical services directly related to area of clinical responsibility or privileges.• Shall attend and participate in meetings, professional staff conferences and other appropriate professional activities such as, but not limited to the following: quality improvement meetings, professional staff meetings, commander's staff meetings, flight meetings, and other meetings required by applicable regulations, MTF guidance, or as directed by the flight Commander or his/her designated representative.• Shall possess knowledge of professional care theories, principles, practices, and procedures to perform nursing assignments of moderate difficulty and experience in assessing pediatric, adolescent, and adults as appropriate. Utilizes the nursing process as a basis for professional practice.• Shall possess basic knowledge of a wide range of medical disorders/conditions and disease processes across the lifespan, as appropriate.• Shall possess knowledge of preventive health schedules, procedures, processes, and education methods.• Shall possess basic knowledge of disease management and population health principles.• Shall possess knowledge of a variety of pharmacological agents used in patient treatment, the desired effects, side effects, and complications of their use as well as the accurate administration of the pharmacologic agent, including dosage calculations as required. Knowledge of the administrative requirements for proper documentation of patient conditions including disease progress, acknowledgement of teaching, and follow up care.• Shall possess basic knowledge of operating a wide variety of medical equipment/non-medical equipment to ensure patient and staff safety, proper equipment maintenance and accessibility.• Shall possess basic knowledge of the administrative requirements for proper documentation of patient’s condition including disease progress, acknowledgement of teaching, and follow up care. • Shall possess ability to acquire and use basic computer skills to enter patient data and extract patient’s information, from a variety of sources to include Genesis, Essentris, ASIMS, AMWITS, Humana TRICARE, and Integrated Clinical Database (ICDB).• Make referral appointments and arrange specialty care as appropriate. Qualifications Minimum Qualifications: * Degree/Education: Shall be a graduate from an accredited school of nursing and have taken NCLEX-PN boards and passed. * Experience: Employment in the nursing field within the last 12 months is mandatory and at least 2 years of active clinical nursing practice in an inpatient environment. * Licensure: Active/unrestricted LPN/LVN license from any state. * Life Support Certifications: Possess current ARC or AHA BLS certification. Sugar, Temperature, Airway, Blood pressure, Lab work, Emotional support program (S.T.A.B.L.E) and Neonatal Resuscitation Program (NRP) will need to be completed within 6 months of hire. * Certification : Contractor must provide documentation/certification of IV therapy training from one or more of the following educational alternatives: Post-graduate level course, Credit for Previous Education or Nontraditional Education * Security: Possess ability to pass a background check/security clearance.
