Job Summary: Under the supervision of the agency's Clinical Supervisor, the Home Health Aide will serve as a member of the care team, working under the supervision of a registered nurse or therapist. The Home Health Aide will be responsible for performing various personal care services as necessary to meet the patient's needs. The Home Health Aide is responsible for observing patients, reporting these observations and documenting observations as well as any care performed. The Home Health Aide will be assigned in a manner that promotes quality, continuity and safety in patient care delivery.
