Description

The Clinical Program Manager is responsible for assuring compliance with mandated clinical orientation, competency assessment and interventions in the assigned clinical program of designated nursing staff within the Medical Center.   This individual interfaces directly with Mount Sinai Health System Departments, Administrators, Nursing, Medical and Quality Leadership, and Clinical Staff, managing the educational and operational impact of all clinical care delivery, staff performance, and outcome measures to support clinical practice


Responsibilities

Responsible For
Development, implementation, coordination and evaluation of clinical programs and services related to core measures.
Principal Duties and Responsibilities
Clinical Leadership
1.    In conjunction with Nursing, Medical and Quality Leadership, coordinates core measures and clinical intervention compliance activities.
2.    Assists in development and design of action plans to support compliance with measures indicators and provides support and educational resources to departments and services relative to core measures quality assessment and improvement processes.
3.    Assists in establishing and implementing short and long term goals, objectives, policies and operating procedures for core measures programs.
4.    Provides coordination and leadership in the execution of day to day program activities, as appropriate to program objectives and areas of expertise.
5.    Oversees the collection, compilation, and analysis of clinical program activity data.
6.    Provides technical assistance, resource services, consultation and education to clinical departments relative to core measures activities.
7.    Participates in and conducts process improvement analysis where applicable.
8.    Responsible for extension of the program in terms of new services or areas.
9.    Promotes an interdisciplinary approach in patient care delivery.
10.    Serves as a resource for patients and their families.
11.    Acts as a patient advocate utilizing the Patient Bill of Rights.
12.    Participates in promoting and maintaining a safe and therapeutic environment.

Administrative Practice
1.    Prepares reports and analysis associated with the institutions core measures programs i.e. senior management reports, departmental and clinical service program evaluation, quarterly reports, etc.
2.    Facilitates cooperation within the program between all departments, services and units.
3.    Provides timely status reports of results of quality monitoring relative to areas that are assigned including identification of patterns and trends that require actions.
4.    Collaborates with other members of the health care team in the development, implementation, and ongoing review of policies, procedures, and standards of care for designated patient populations.
5.    Demonstrates knowledge of hospital, departmental and care center standards, policies, procedures and guidelines as well as regulatory agency standards. Assures readiness for Joint Commission and other agency surveys and presents related programs during Joint Commission and other agency visits.
6.    Participates in the development and implementation of continuous quality improvement programs.
7.    Participates in quality management activities including identifying and communicating issues of performance improvement and risk in a timely manner.
8.    Participates in committee activities.
9.    Is aware of and responsive to budgetary constraints and incorporates financial principles into program development and allocation of resources.
10.    Recognizes each individual's right to health care unrestricted by age, race, color, sex, age, national origin, disability, sexual orientation or source of payment.
11.    Assists in the performance appraisal process of designated Nursing staff.
12.    Researches opportunities for use in benchmarking for assigned areas.
 


Qualifications
  • Baccalaureate Degree in Nursing is required a Master?s in Nursing is required 
  • Progressive job-related experience specific to area of responsibility
  • New York State License with current registration as a registered professional nurse, nurse practitioner preferred.

The salary range for the role is $115494 - $173241 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. Job Description Looking for a Nurse Manager for Cardiac Access/Cardiac Observation who is responsible for leading and implementation program that supports care for the adult cardiac patient. The Unit provides care across the cardiac outpatient, extended recovery and inpatient population primarily pre and post procedure. This manager will support orientation and competency assessment of all staff, promote standardized clinical workflow and optimal patient care. The manager evaluates performance, provides feedback and mentors, develop education programs, and contribute to staff professional development. Working closely to advocate for the CAU team with cardiac procedural areas, physicians and patients. Principal Accountabilities Identifies the clinical and professional development needs of nursing staff, plans and provides education in area of expertise. Develops and provides educational programs and conferences for staff, other departments, and community. Plans and implements quality improvement programs for both clinical education and environment of care. Works with staff to identify interventions needed to successfully implement workflow changes. Reviews patient and staff data to measure the effectiveness of patient care. Works with senior leadership, interdisciplinary teams and other stakeholders in order to develop, implement and evaluate programs and services in align with the system. Addresses issues to promote a productive and supportive work environment. Maintains clinical and non-clinical guidelines by writing and updating policies and procedures. Promotes and maintains safe and clean working environment by designing and implementing procedures, rules, and regulations. Provides assistance from other health care professionals within the system and from regulatory agencies.
