Core Job Functions
Refers patients to correct laboratory, radiology center, hospital or specialist. Ensures precertification is obtained when needed prior to services being rendered. Follows referral process based upon insurance company's policies and procedures.
Provide guidance on available services and self management. Assist patients in navigating health education and supportive services within and outside of clinic, including the facilitation of referrals. Follow-up on care and ensure that patients are receiving appropriate care.
Administers medication safely. Provides quality care to all patients. Takes initiative and uses sound judgment.
Follow policies and procedures for routine office procedures, test, administration of medications and assisting providers up to and including IV therapy. Recognizes and anticipates urgent and emergent patient situations and responds appropriately.
Identifies patient education needs and may assist in patient teaching as to appropriate health maintenance and management of illness. Communicates effectively with patients, families, and other members of the health care team.
Informs patients on physician instructions and any preparations. Informs patients about procedures prior to performing procedures. Explains consent forms to patients and has patients sign consent forms.
Vital signs are taken on each patient, on each visit and input into eClinical Works. Verifies and updates patients clinical record on each visit.
Position Summary Under the general direction of the Center for Medication Management Clinic Manager, provide advanced education and care to patients receiving anticoagulant therapy, hyperlipidemia management, and diabetic education. Responsible for working with physicians within the predefined guidelines approved by the P&T Committee for management of patients referred for anticoagulation therapy, hyperlipidemia management, and diabetic education. This includes but is not limited to the following medications warfarin (Coumadin), low molecular weight heparin (LMWH), aspirin, simvastatin (Zocor), lovastatin (Altoprev, Mevacor), rosuvastatin (Crestor), fluvastatin (Lescol), atorvastatin (Lipitor), pitavastatin (Livalo), and pravastatin (Pravachol). Other medications include bile acid resins: cholestyramine, colestipol, colesevelam, and the antilipemic agent: eztimibe (Zetia). Niacin, Fish oil, and fibric acid derivatives: Fenofibrate, gemfibrozil, and fenofibric acid are several other medications. The nurse practitioner is responsible directly for the warfarin (Coumadin) dosing, patient education, scheduling of appointments, identifying drug-drug interactions and other responsibilities related to issues that arise in the management of patients’ warfarin (Coumadin) therapy, such as bleeding and scheduled procedures. The nurse practitioner is also responsible for managing a patient’s basic or advanced lipid profile by medication management and lifestyle modification. The nurse practitioner provides education and other responsibilities involving lipid management. The nurse practitioner also provides detailed diabetic education, including but not limited to, information about drug regimen, diet, and exercise. Responsible for providing tools and educational opportunities to enhance services provided by all staff members. Serves as assistant preceptor to students and residents.
