Facilitates proactive and excellent communication within the patient’s multidisciplinary team, which includes but is not limited to, the managing physicians, patients, precertification department, and treatment facilities. Addresses barriers to care through appropriate referrals to applicable supportive care services. Barriers range from psychosocial and educational to transportation and financial.
