Job Summary
Leads and supports initiatives to maximize throughput and capacity in the acute care space. Monitor timely transitions of care across the continuum to include Emergency Department, Inpatient units, and Procedural Areas.Essential Functions

Ensures standardization of processes for inpatient transfers and patient discharges.
Monitors and evaluates throughput metrics, in collaboration with campus leadership teams, to develop action items to address throughput opportunities and improve processes.
Collaborates with Providers, Care Management, Nursing and other members of the care team to ensure throughput barriers are minimized and elevated as appropriate.
Creates and executes plans to ensure smoothing of patient throughput, length of stay management, and other efforts to deliver continuous, predictive, and optimal bed capacity management.
Supports efforts to improve patient throughput and ensure safe, timely care transitions for our patients across the Market.
Collaborates with Transfer Center, Providers, Emergency Department, Inpatient Units, Administrators on Duty, EMS Transports, Environmental Services, and other support services to improve care transitions.

Supervisory/Management Responsibilities
This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Requirements
Education - Associate degree in Nursing. Bachelor’s degree in nursing preferred.
Experience - Two (2) years of experience as a registered nurse.

Holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working.
