The Palliative Medicine Program at Jefferson Einstein Philadelphia Hospital is seeking a motivated and compassionate Nurse Practitioner/Physician Assistant to join our interdisciplinary team. This is an exciting opportunity to practice high-impact palliative care in a dynamic academic medical center, caring for patients with complex, serious illnesses across a variety of inpatient settings, with opportunities for ambulatory practice, teaching, and academic involvement.
About the Role
This position is primarily focused on inpatient palliative care consultation, including care of medically complex patients throughout the hospital. The Nurse Practitioner/Physician Assistant will work closely with palliative medicine attending physicians and an experienced interdisciplinary team to provide expert symptom management, support patients, and families through challenging medical decisions, and enhance quality of care for individuals with serious illness. The role also includes participation in ambulatory palliative care clinics, offering continuity and longitudinal care when appropriate. This role will see adults of ALL ages (not just the geriatric population)
Key Responsibilities
• Provide inpatient palliative care consultation for patients with a broad range of serious and chronic illnesses
• Care for patients across multiple hospital settings, including ICUs, medical and surgical units, and step-down units
• Conduct comprehensive assessments addressing symptom burden, goals of care, psychosocial needs, and treatment preferences
• Partner closely with the palliative medicine attending physician in shared clinical decision-making
• Participate in complex symptom management, including pain and non-pain symptoms
• Facilitate goals-of-care discussions and advance care planning with patients and families
• Participate in ambulatory palliative care clinics, providing outpatient consultation and follow-up
• Collaborate with an interdisciplinary team including physicians, social workers, chaplains, nurses, and trainees
• Communicate effectively with primary teams and consultants to ensure coordinated care
• Document clinical care clearly and efficiently in the electronic medical record

- Engage in the academic mission of an established palliative care program within a teaching hospital
Qualifications
Required:
- Graduate of an accredited Acute Care Nurse Practitioner program or PA School
- Active Pennsylvania Acute Care Nurse Practitioner/Physician Assistant license or eligibility
- Comfort working with patients with serious illness in inpatient hospital settings
- Strong communication skills and ability to support patients and families during complex decision-making
Preferred:
- Prior experience in palliative care/emergency medicine/critical care is preferred
- Acute Care Nurse Practitioner certification or NCCPA Certification
- Experience working in ICU and high-acuity inpatient environments
